基于数据挖掘技术探讨中医外治法治疗中枢性呃逆的选穴规律

Rules of Acupoints Selection for Treating Intractable Hiccup by Traditional Chinese Medicine Based on Data Mining

目的:运用数据挖掘技术探讨中医外治法治疗中枢性呃逆的选穴规律。方法:以相关中英文主题词,从中国知网(CNKI)、中文科技期刊数据库(维普)、中国生物医学文献数据库(CBM)、万方数据知识服务平台检索并下载相关文献,阅读并筛选文献后摘录处方,形成数据库,通过SPSS 25.0软件进行高频选穴、特定穴、分布部位、归经频次、关联规则和系统聚类分析。结果:纳入文献272篇,处方296首,腧穴111个,总频次为1 462次,涵盖毫针、电针、皮内针、灸法、穴位按摩等疗法。高频选穴由高到低排列为足三里、内关、中脘、膈俞、攒竹;选穴归经以任脉、足阳明胃经、手厥阴心包经、足太阳膀胱经等为主。特定穴通常选用五输穴、八会穴、络穴等。选穴主要分布在下肢。高频选穴组合为内关+足三里,内关+中脘,足三里+中脘。结论:中医外治法治疗中枢性呃逆选穴多从顾护胃气、和畅血行的角度考虑,可为临床提供切实可行的治疗思路。

Objective:To explore rules of acupoints selection for treating intractable hiccup by traditional Chinese medicine(TCM)using data mining.Methods:According to Chinese and English subject terms,relevant literatures from China National Knowledge Infrastructure(CNKI),Chinese Science and Technology Journal Database(VIP),China Biomedical Literature Database(CBM),and Wanfang Data knowledge service platform were searched and to form the database,SPSS 25.0 software was used to carry out high frequency acupoints selection,specific acupoints,distribution locations,channel reduction frequency,association rules,and systematic cluster analysis.Results:Two hundred seventy-two literatures,296 prescriptions,and 111 acupoints were included,with 1462 times.The millimeter acupuncture,electric acupuncture,intradermal acupuncture,moxibustion,acupressure,and other therapies were included.The high frequency acupoint were:Zusanli,Neiguan,Zhongwan,Geshu,Zangzhu.The main points were stomach meridian of foot-yangming,pericardium meridian of hand-jueyin and bladder meridian of foot-taiyang.Five-shu points,eight-influential points and luo-connecting points were usually selected.The acupoints were mainly situated on the lower limbs.The combination of high frequency points were Neiguan+Zusanli,Neiguan+Zhongwan and Zusanli+Zhongwan.Conclusion:The treatment of intractable hiccup by TCM is mainly focused on the protecting stomach Qi and promoting blood circulation,which can provide practical treatment ideas for clinic.
