尘肺病患者血清中Cys-C水平及影响因素的研究

Serum Cys-C level and its influencing factors in patients with pneumoconiosis

摘要 目的研究尘肺病患者血清中胱抑素C(Cystatin C,Cys-C)的水平,并探究血清Cys-C与尘肺病患者肺功能及炎症指标的相关性。方法随机选取2022年1-12月于本院就诊的261例尘肺病患者为尘肺组,选取同期门诊健康体检者77例为对照组,研究对象均完成肺功能、血清中Cys-C、超敏C反应蛋白、白细胞及肌酐等检测。结果与对照组比较,尘肺组血清中Cys-C及超敏C反应蛋白水平明显升高(Z=-2.66、-5.27,P<0.01),第1秒用力呼气容积占预计值百分比(FEV_(1.0)%)、1秒率(FVE_(1.0)/FVC)及一氧化碳弥散量占预计值百分比(DLCO%)明显降低(t=6.02、5.43、12.30,P<0.01),血清中Cys-C水平与血清中肌酐和超敏C反应蛋白呈正相关(r=0.45、0.33,P<0.01),与FEV_(1.0)%呈负相关(r=-0.59,P<0.01)。结论血清中Cys-C水平与老年尘肺病患者的肺功能及炎症因子水平相关,为尘肺病的进一步研究提供理论依据。 Objective The aims of the present study were to determine the level of serum cystatin C in patients with pneumoconiosis and to investigate the relationships between cystatin C levels with pulmonary function and inflammation parameters.Methods From January 2022 to December 2022,261 patients with pneumoconiosis were randomly selected from our hospital as pneumoconiosis group,during the same period,a control group of 77 healthy outpatients was recruited for this study.Pulmonary function tests,serum cystatin C,high-sensitivity C-reactive protein(hs-CRP),serum creatinine,and blood leukocyte levels were measured in all participants.Results The pneumoconiosis group displayed significantly higher serum cystatin C values(Z=-2.66,P<0.01)and high-sensitivity C-reactive protein(Z=-5.27,P<0.01),and substantially lower predicted forced expiratory volume in 1 s(FEV_(1.0)%)(t=6.02),predicted diffusion lung capacity for carbon monoxide(t=12.30)and forced expiratory volume in 1 s/forced vital capacity(t=5.43)than the healthy control group(P<0.01).The serum cystatin C level were positively correlated with the serum creatinine level(r=0.45,P<0.01)and hs-CRP level(r=0.33,P<0.01),and a significant negative association was found between serum cystatin C levels and the predicted FEV_(1.0)%(r=-0.599,P<0.01).Conclusion Serum cystatin C levels may be associated with lung function decline and inflammation in elderly patients with pneumoconiosis.Serum cystatin C levels are likely to provide a theoretical basis for further study of pneumoconiosis.
