25-(OH)D、糖化血红蛋白检测对2型糖尿病合并高血压患者的临床价值

Clinical value of 25-(OH)D and hemoglobin A1c detection in type 2 diabetes mellitus with hypertension

目的分析2型糖尿病(T2DM)合并高血压患者血清25-羟基维生素D[25-(OH)D]、糖化血红蛋白(HbA1c)表达及其与T2DM合并高血压的关系。方法回顾性选取2022年1月—2023年1月河南省第二人民医院接收的107例T2DM患者,分为两组A组(T2DM合并高血压患者)59例、B组(单纯T2DM)48例,同时选取同时期健康志愿者62例(计为C组)作为对照。结果A组血清25-(OH)D水平低于B组、C组(P<0.05),B组25-(OH)D浓度低于C组(P<0.05),而A、B、C组的血清HbA1c水平依次降低(P<0.05);25-(OH)D、HbA1c血清浓度与T2DM患者是否合并高血压相关(P<0.05)。ROC分析显示,当25-(OH)D<26.12μg/L、HbA1c>8.74%时,T2DM患者易合并高血压,两者联合预测(串联模型)的AUC最大(为0.801)(P<0.05)。结论合并高血压的T2DM患者血清25-(OH)D会呈现一定程度的降低、HbA1c水平升高,对于25-(OH)D、HbA1c出现异常变化的T2DM患者需要提高警惕,及时采取措施,避免高血压的发生。

Objective To explore the expression of 25-hydroxyvitamin D[25-(OH)D]and hemoglobin A1c(HbA1c)in serum of patients with type 2 diabetes mellitus(T2DM)and hypertension.Methods This study retrospectively reviewed 107 patients with T2DM admitted to the hospital between January 2022 and January 2023.Among them,59 patients with hypertension were included in group A,and 48 patients with simple T2DM were included in group B.Meanwhile,62 healthy volunteers were selected as control(group C).Results Serum 25-(OH)D level increased in order,and serum HbA1c level decreased in order from group A,group B to group C(P<0.05).Serum levels of 25-(OH)D and HbA1c were related to hypertension in patients with T2DM(P<0.05).ROC analysis showed that when 25-(OH)D level was lower than 26.12μg/L and HbA1c was higher than 8.74%,patients with T2DM were more likely to have hypertension.The AUC of joint prediction with the two indicators(series model)was the largest(0.801)(P<0.05).Conclusion Patients with T2DM and hypertension have decreased serum 25-(OH)D level and increased HbA1c.Attention should be paid to T2DM patients with abnormal changes in 25-(OH)D and HbA1c to take measures in time,thereby avoiding the occurrence of hypertension.
