根因分析法结合PDCA循环在中药调剂工作中的应用效果

Application effect of root cause analysis combined with PDCA cycle in TCM dispensing

目的探讨根因分析法结合PDCA循环在中药调剂工作中的应用效果。方法选取2021年1月—2021年12月在平顶山市中医医院使用常规安全管理模式的中药处方10000张作为对照组,选取2022年11月—2022年12月在平顶山市中医医院使用根因分析法结合PDCA循环管理模式的中药处方10000张作为研究组,比较两组调配失误情况、取药时间、投诉事件、不良反应以及取药满意度,并分析调剂失误原因。结果研究组错配率、多配率、漏配率、调配失误率分别为0.06%、0.06%、0.03%、0.15%,明显低于对照组的0.29%、0.25%、0.27%、0.81%(P<0.05)。两组调剂失误原因主要由品种差错、剂型差错。研究组取药时间明显短于对照组(P<0.05),研究组投诉事件、不良反应发生率分别为4.40%、5.40%,明显低于对照组的11.80%、10.20%(P<0.05)。研究组取药满意度优于对照组(P<0.05),研究组取药总满意度为99.45%,明显高于对照组的97.60%(P<0.05)。结论根因分析法结合PDCA循环应用在中药调剂工作中,可有效降低调配失误率、投诉事件、不良反应,提高取药满意度。

Objective To explore the application effect of root cause analysis combined with PDCA cycle in TCM dispensing.Methods A total of 10,000 TCM prescriptions using conventional safety management mode in Pingdingshan Traditional Chinese Medicine Hospital from January to December 2021 were selected as the control group,and 10,000 TCM prescriptions using root cause analysis combined with PDCA cycle management mode in our hospital from November to December 2022 were selected as the study group.The dispensing errors,drug taking time,complaints,adverse reactions and drug taking satisfaction of the two groups were compared,and the causes of dispensing errors were analyzed.Results The mismatching rate,multiple matching rate,missing matching rate and dispensing error rate of the study group were 0.06%,0.06%,0.03% and 0.15%,respectively,which were significantly lower than those of the control group(0.29%,0.25%,0.27% and 0.81%,P<0.05).The reasons for the two groups of dispensing errors are mainly variety errors and dosage form errors.The drug taking time in the study group was significantly shorter than that of the control group(P<0.05),and the incidence of complaint events and adverse reactions in the study group were 4.40%and 5.40%,respectively,which were significantly lower than those of the control group(11.80% and 10.20%,P<0.05).The satisfaction of the study group was better than that of the control group(P<0.05),and the total satisfaction of the study group was 99.45%,which was significantly higher than that of the control group(97.60%,P<0.05).Conclusion The application of root cause analysis combined with PDCA cycle in TCM dispensing can effectively reduce the dispensing error rate,complaints,adverse reactions,and improve the satisfaction of taking medicine.
