单操作孔与三孔胸腔镜肺癌根治术对术后疼痛和免疫功能的影响

Effect of single-hole and three-hole thoracoscopic radical resection of lung cancer on postoperative pain and immune function

目的探究单操作孔与三孔胸腔镜肺癌根治术对术后疼痛和免疫功能的影响。方法选取我院2019年7月至2021年7月收治的68行胸腔镜肺癌根治术的患者,根据手术方式进行分组,将手术时实施单操作孔的34例患者设为观察组,将手术时实施三孔的34例患者设为对照组。比较两组患者术后第3d、第5d疼痛情况及免疫功能情况。结果术后第3d、第5d观察组患者VAS评分分别为(3.31±1.13)分、(1.36±0.76)分,均低于对照组术后第3d(4.41±1.21)分、术后第5d(2.54±0.86)分(P<0.05);术后第3d观察组血清CD3+(59.44±6.78)%、CD4+(31.27±6.48)%均高于对照组血清CD3+(51.36±6.15)%、CD4+(25.11±6.23)%(P<0.05),观察组血清CD8+(26.31±5.94)%低于对照组血清CD8+(32.45±5.83)%(P<0.05)。结论单操作孔手术方式治疗安全性更高,可有效减轻患者术后疼痛,减少对患者机体免疫功能的破坏,值得临床推广。

Objective To investigate the effect of single-hole and three-hole thoracoscopic radical resection of lung cancer on postoperative pain and immune function.Methods Sixty-eight patients who underwent thoracoscopic radical resection of lung cancer in our hospital from July 2019 to July 2021 were selected as the study subjects.According to surgical methods,34 patients who underwent single-hole surgery were selected as the observation group,and 34 patients who underwent three-hole surgery were selected as the control group.The pain and immune function were compared between the two groups on the 3rd and 5th day after operation.Results The VAS scores on the 3rd and 5th day after operation in the observation group were(3.31±1.13)points and(1.36±0.76)points,respectively,which were lower than those on the 3rd day after operation[(4.41±1.21)points]and the 5th day after operation[(2.54±0.86)points]in the control group(P<0.05).The serum CD3+(59.44±6.78)%and CD4+(31.27±6.48)%on the 3rd day after operation in the observation group were higher than those in the control group[(51.36±6.15)%and CD4+(25.11±6.23)%](P<0.05).The serum CD8+(26.31±5.94)%in the observation group was lower than that in the control group[(32.45±5.83)%](P<0.05).Conclusion Single-hole surgery is safer,which can effectively reduce postoperative pain and reduce the damage to the immune function of patients.It is worthy of clinical promotion.
