体外冲击波疗法联合牛痘疫苗接种家兔炎症皮肤提取物对带状疱疹后神经痛患者疼痛程度及生活质量的影响

Effects of extracorporeal shock wave therapy combined with vaccinia vaccine inoculation of rabbit inflammatory skin extract on pain degree and quality of life of patients with post herpetic neuralgia

目的观察牛痘疫苗接种家兔炎症皮肤提取物(恩再适)联合体外冲击波疗法治疗带状疱疹后神经痛(PHN)患者的治疗效果。方法根据治疗方式不同前瞻性将我院2019年5月至2021年7月收治的PHN患者94例分为观察组、对照组,各47例。对照组给予颈部椎旁神经阻滞+恩再适治疗,在对照组治疗基础上观察组加用体外冲击波治疗。对比两组临床疗效、临床症状改善情况、疼痛程度、生活质量及不良反应发生率。结果观察组治疗总有效率93.62%高于对照组78.72%(P<0.05);治疗2周后,观察组疼痛程度较对照组降低,生活质量较对照组提高(P<0.05);治疗2周后,两组麻木、局部疼痛、痛觉敏感症状评分均降低,且观察组低于对照组(P<0.05);观察组不良反应发生率8.51%与对照组10.64%比较,无统计学意义(P>0.05)。结论在恩再适基础上加用体外冲击波,可改善PHN患者临床症状,减轻其疼痛程度,提高患者生活质量,疗效显著,具有一定安全性。

Objective To observe the therapeutic effect of rabbit inflammatory skin extract(ENZ)inoculated with cowpox vaccine combined with extracorporeal shock wave therapy on post herpetic neuralgia(PHN).Methods According to different treatment methods,94 patients with PHN admitted in our hospital from May 2019 to July 2021 were prospectively divided into the observation group and the control group,47 patients in each group.The control group was treated with cervical paravertebral nerve block+ENZ,and the observation group was treated with extracorporeal shock wave on the basis of the control group.The clinical efficacy,improvement of clinical symptoms,degree of pain,quality of life and incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 93.62%higher than that of the control group(78.72%,P<0.05);After 2 weeks of treatment,the pain level in the observation group was lower than that in the control group,and the quality of life was higher than that in the control group(P<0.05);After 2 weeks of treatment,the scores of numbness,local pain and pain sensitivity symptoms in both groups decreased,and the scores in the observation group were lower than those in the control group(P<0.05);The incidence of adverse reactions in the observation group was 8.51%compared with 10.64%in the control group,with no statistical significance(P>0.05).Conclusion The use of extracorporeal shock wave on the basis of ERZ can improve the clinical symptoms of PHN patients,relieve their pain,and improve the quality of life of patients.It has a significant effect and a certain degree of safety.
