康复前移理念指导下的吞咽康复训练对声门型喉癌患者吞咽功能、误吸风险及进食安全性的影响

Influence of swallowing rehabilitation training guided by the concept of rehabilitation forward on swallowing function,risk of aspiration and eating safety in patients with glottic laryngeal carcinoma

目的分析声门型喉癌患者应用康复前移理念指导下的吞咽康复训练对吞咽功能、误吸风险及进食安全性的影响。方法选取2020年10月至2022年3月期间我院收治的87例声门型喉癌术后患者作为研究对象,采用随机数字表法分组。常规组43例给予常规吞咽康复训练,研究组44例给予康复前移理念指导下的吞咽康复训练。对比两组患者吞咽功能、误吸风险及进食安全性。结果研究组患者口腔期、咽喉期评分低于常规组,视频透视吞咽检查(VFSS)评分高于常规组(P<0.05);研究组患者渗透-误吸评分(PAS)、Rosenbek渗漏/误吸量表期分值均低于常规组(P<0.05);研究组噎食、口鼻反流、呛咳总发生率低于常规组(P<0.05)。结论康复前移理念指导下的吞咽康复训练可有效提高声门型喉癌术后患者吞咽功能及进食安全性,降低误吸风险,可在临床推广应用。

Objective To analyze the effect of swallowing rehabilitation training under the guidance of the concept of rehabilitation advancement on swallowing function,aspiration risk and eating safety in patients with glottic laryngeal carcinoma.Methods A total of 87 postoperative patients with glottic laryngeal carcinoma who were treated in our hospital from October 2020 to March 2022 were selected as the research subjects,and grouped by random number table method.43 cases in the routine group were given routine swallowing rehabilitation training,and 44 cases in the study group were given swallowing rehabilitation training under the guidance of the concept of rehabilitation forward.The swallowing function,the risk of aspiration and the safety of eating were compared between the two groups.Results The oral stage and throat stage scores of the study group were lower than those of the routine group,and the video fluoroscopy swallowing examination(VFSS)score was higher than that of the routine group(P<0.05).The scores of the aspiration scale were lower than those of the conventional group(P<0.05);the total incidence of choking,nasal reflux and choking in the study group were lower than those in the conventional group(P<0.05).Conclusion Swallowing rehabilitation training guided by the concept of rehabilitation advancement can effectively improve the swallowing function and eating safety of patients with glottic laryngeal carcinoma after surgery,reduce the risk of aspiration,and can be popularized and applied in clinical practice.
