Reconstructive three-dimensional tissue Doppler imaging for quantifying left ventricular myocardial infarction area after coronary artery occlusion: an in vivo chronic animal study

摘要 目的 通过活体动物实验探讨三维重建组织多普勒超声成像测量心肌梗死面积的方法和可行性。方法 用三维重建组织多普勒显像的方法 ,对 8只开胸的心尖部心肌梗死和室壁瘤的活体羊模型在 4种不同的血流状态下进行心肌梗死面积的测量。结果 重建三维超声组织多普勒成像测量的活体羊心内膜梗死的面积与尸检标本测量的面积密切相关 (r =0 .90 ,Y =0 .83X +1 .33 ,s x =0 .92cm2 ,P <0 .0 0 0 1 )。结论 重建三维组织多普勒成像提供了一系列动态空间组织多普勒数据确定室壁异常运动区 ,并能定量测量心肌梗死面积 ,在缺血性心脏疾病中 。 Objective To test a method of estimating myocardial infarction area of letf ventricular(LV) by constructive three dimensional tissue Doppler imaging (3D TDI) in vivo chronic animal study. Methods Eight sheep underwent occlusion of LAD coronary artery or diagonal branches to create apical myocardial infarction and aneurysm formation, at which time 3D TDI echo images were obtained on open chest anesthetized animals. At follow up study, constructive three dimensional tissue Doppler imaging were used to measure myocardial infarction area of the LV in 4 hemodynamic conditions for each sheep.Results 3D TDI calculated endocardial infarcted segment areas correlated closely with measurements on the postmortem heart specimens after triphenyl tetrazolium chloride (TTC) staining (r= 0.90 ,Y= 0.83 X+ 1.33 ,s = 0.92 cm 2, P< 0.000 1 ). Conclusions Reconstructive 3D TDI provides robust dynamic spatial tissue Doppler data to identify areas with abnormalities of regional wall motion and could quantify LV myocardial infarction areas. It may aid noninvasive functional assessment in ischemic heart disease.
