摘要
目的探讨PDCA循环结合模拟考核在儿科专业基地住院医师规范化培训(简称住培)结业临床实践能力考核的应用。方法选择该院儿科专业基地参加住培结业实践技能考试的26名住院医师作为研究对象。在考前采取模拟考试、模拟考核结果反馈和座谈,发现住培管理中两个薄弱环节:平时缺乏病历书写的锻炼,在规定时间内很难完成病历书写;临床病例分析不完善,在接诊患者和临床病例分析方面的能力需要提高。要求儿科专业基地住院医师每月书写大病历2份,必须有指导医师修改痕迹。采取PDCA(计划、执行、检查、处理)循序持续改进。结果该院儿科专业基地住培结业实践技能考核通过率100.00%。结论运用PDCA循环结合模拟考核利于儿科专业基地住院医师结业实践技能考核。
Objective To explore the application of PDCA cycle combined with simulation assessment in standardized training for resident physicians in our pediatric professional base.Methods This study selected 26 resident physicians from pediatric professional base who participated in the residency training graduation practical skills exam as the research subjects.Before the exam,simulated exams,feedback on simulated assessment results,and discussions were conducted,and two weak links in residency training management were found.First,they were lack of exercise in case writing in daily life,making it difficult to complete case writing within the specified time frame.Second,the clinical case analysis was incomplete,and the ability to receive patients and analyze clinical cases needed to be improved.Resident physicians in pediatric professional bases were required to write 2 copies of major illness cases per month,and there must be traces of guidance from physicians for revision.This paper adopted PDCA(plan,execute,inspect,and handle)for continuous improvement in a sequential manner.Results The passing rate of standardized training for pediatric residents and completion skills assessment was 100%.Conclusion PDCA cycle combined with simulation assessment is helpful for pediatric professional bases to complete standardized training of residents and pass the skills assessment.
