射频消融介入术联合GEMOX方案治疗肝内胆管癌患者的效果

Efficacy of Radiofrequency Ablation Combined with GEMOX Chemotherapy for Patients with Intrahepatic Cholangiocarcinoma

目的探究射频消融介入术联合GEMOX方案化疗治疗肝内胆管癌(ICC)患者的效果。方法选取郑州市中医院肿瘤介入科2022年1月至2023年1月收治的100例ICC患者,根据治疗方式分为对照组(50例)与观察组(50例),对照组接受射频消融术治疗,观察组接受射频消融术联合GEMOX方案,观察两组临床疗效以及并发症发生情况,比较两组血清环氧合酶-2(COX-2)、趋化因子9(CXCL9)、白细胞介素-6(IL-6)、血清皮质醇(Cor)、去甲肾上腺素(NE)、谷草转氨酶(GOT)、谷丙转氨酶(GPT)、总胆红素(T-Bil)水平。结果观察组治疗有效率高于对照组(P<0.05),观察组COX-2、CXCL9、GOT、GPT、T-Bil水平低于对照组(P<0.05),IL-6、Cor、NE水平高于对照组(P<0.05)。观察组感染、胆道出血、胆汁漏、胆道梗阻并发症的总发生率低于对照组(P<0.05)。结论射频消融介入术联合GEMOX方案治疗ICC具有显著优势,可能通过降低COX-2和CXCL9水平改善肿瘤的免疫环境,具有一定的肝脏保护作用,有助于减轻肝损伤和改善肝功能,降低并发症发生率,提高患者的生活质量。

Objective In order to explore the efficacy of radiofrequency ablation combined with GEMOX chemotherapy in the treatment of patients with intrahepatic cholangiocarcinoma(ICC).Methods A total of 100 patients with ICC admitted to the Department of Oncology Intervention of Zhengzhou Hospital of Traditional Chinese Medicine from January 2022 to January 2023 were selected and divided into control group(50 cases)and observation group(50 cases)according to the treatment method.The control group was treated with radiofrequency ablation,and the observation group was treated with radiofrequency ablation combined with GEMOX program.The clinical efficacy and complications of the two groups were observed,and the serum cyclooxygenase-2(COX-2),chemokine C motif motif 9(CXCL9)levels,interleukin-6(IL-6),serum cortisol(Cor),norepinephrine(NE),glutamic-oxaloacetic transaminase(GOT),glutamic-pyruvic transaminase(GPT)and total bilirubin(T-Bil)levels of the two groups were compared.Results The effective rate of treatment in observation group was higher than that in control group(P<0.05).The levels of COX-2,CXCL9,GOT,GPT and T-Bil in observation group were lower than those in control group(P<0.05),and IL-6,Cor,NE levels were higher than those of control group(P<0.05).The total incidence of complications of infection,biliary bleeding,bile leakage and biliary obstruction in observation group was lower than that in control group(P<0.05).Conclusion Radiofrequency ablation combined with GEMOX has significant advantages in the treatment of ICC patients,which may improve the immune environment of tumors by reducing the levels of COX-2 and CXCL9,and has a certain liver protection effect,which helps to reduce liver damage and improve liver function,reduce the incidence of complications,and thus improve the quality of life of patients.
