星状神经节阻滞联合舍曲林治疗卒中后抑郁及失眠的效果

Effects of Stellate Ganglion Block Combined with Sertraline in the Treatment of Post Stroke Depression and Insomnia

目的探讨星状神经节阻滞联合舍曲林治疗卒中后抑郁及失眠的效果。方法前瞻性纳入医院2020年8月至2022年8月收治的80例卒中后抑郁及失眠患者为研究对象,采用奇偶数法分为联合组与对照组,各40例。对照组接受舍曲林治疗,联合组接受星状神经节阻滞联合舍曲林治疗,均治疗2个月,治疗结束后随访1个月。对比两组临床疗效,对比两组患者治疗前、治疗2个月、随访结束时睡眠质量、抑郁状况、日常生活能力。结果治疗2个月,联合组治疗总有效率高于对照组(P<0.05);治疗前、治疗2个月及随访结束时,两组匹兹堡睡眠质量指数(PSQI)、蒙哥马利抑郁量表(MADRS)评分、日常生活活动能力量表(ADL)评分的组间、时间、交互效应有统计学意义(P<0.05)。结论星状神经节阻滞联合舍曲林治疗卒中后抑郁及失眠患者的效果较好,能够改善患者睡眠质量并消除抑郁情绪,同时提高其日常生活能力。

Objective To explore the clinical efficacy of stellate ganglion block combined with sertraline in the treatment of post stroke depression and insomnia.Methods A prospective study was conducted on 80 patients with post stroke depression and insomnia admitted to the hospital from August 2020 to August 2022.They were divided into combination group and control group using the odd even number method,with 40 patients in each group.The control group received sertraline treatment,while the combination group received stellate ganglion block combined with sertraline treatment.Both groups were treated for 2 months and were followed up for 1 month after treatment.The clinical efficacy of the two groups was compared,and the changes of sleep quality,depression and daily living ability of the two groups before treatment,two months after treatment and at the end of follow-up were compared.Results After two months of treatment,the total effective rate of the combination group was higher than that of the control group(P<0.05).Before treatment,at two months of treatment and at the end of follow-up,the inter group,time,and interaction effects of the Pittsburgh sleep quality index(PSQI),Montgomery-Asberg depression rating scale(MADRS)and activities of daily living(ADL)scores of the two groups were statistically significant(P<0.05).Conclusion The effects of stellate ganglion block combined with sertraline in the treatment of post stroke depression and insomnia is good,which can improve the sleep quality of patients and eliminate depressive emotions,while also improve their daily living ability.
