术前血尿酸与肌酐比值与急性Stanford A型主动脉夹层术后严重出血的关系

Relationship Between Preoperative Blood Uric Acid to Creatinine Ratio and Severe Bleeding After Acute Stanford Type A Aortic Dissection

目的分析术前血尿酸(UA)与肌酐(Cr)比值与急性Stanford A型主动脉夹层患者术后严重出血的关系。方法本研究为回顾性分析,收集新乡医学院第三附属医院2019年6月至2022年10月收治的62例急性Stanford A型主动脉夹层患者的临床资料,根据患者接受孙氏手术后出血情况,将其分为严重出血组(22例)和无严重出血组(40例)。收集两组患者术前血清UA、Cr水平,计算UA/Cr,采用点二列相关性检验术前UA/Cr与急性Stanford A型主动脉夹层患者术后严重出血的关系,采用受试者工作特征(ROC)曲线分析术前UA/Cr对急性Stanford A型主动脉夹层患者术后严重出血的预测价值。结果严重出血组女性占比、术前UA、术前UA/Cr高于无严重出血组(P<0.05)。经点二列相关性分析,术前UA/Cr与急性Stanford A型主动脉夹层患者术后严重出血呈正相关(r=0.468,P<0.001)。经ROC曲线分析,术前UA/Cr值对急性Stanford A型主动脉夹层患者术后严重出血有一定预测价值[曲线下面积(AUC)=0.773,P<0.001]。结论术前UA/Cr值与急性Stanford A型主动脉夹层患者术后严重出血有关,且对术后严重出血的发生具有一定预测价值。

Objective To analyze the relationship between preoperative blood uric acid(UA)to creatinine(Cr)ratio and postoperative severe bleeding in patients with acute Stanford type A aortic dissection.Methods This study was a retrospective analysis,collecting the clinical data of 62 patients with acute Stanford type A aortic dissection admitted to the Third Affiliated Hospital of Xinxiang Medical University from June 2019 to October 2022.According to the postoperative bleeding status of patients undergoing Sun’s surgery,they were divided into a severe bleeding group(22 cases)and a non-severe bleeding group(40 cases).The preoperative serum UA and Cr levels of the two groups of patients were collected,and the UA/Cr was calculated.The relationship between preoperative UA/Cr and postoperative severe bleeding in patients with acute Stanford A Aortic dissection was tested by point binary correlation.The predictive value of preoperative UA/Cr on postoperative severe bleeding in patients with acute Stanford A Aortic dissection was analyzed by receiver operating characteristic(ROC)curve.Results The proportion of women,preoperative UA and preoperative UA/Cr in the severe bleeding group were higher than those in the non-severe bleeding group(P<0.05).By point binary correlation analysis,preoperative UA/Cr was positively correlated with postoperative severe bleeding in patients with acute Stanford type A aortic dissection(r=0.468,P<0.001).Through ROC curve analysis,preoperative UA/Cr ratio had certain predictive value for patients with severe postoperative bleeding[area under the curve(AUC)=0.773,P<0.001].Conclusion Preoperative UA/Cr ratio is associated with severe postoperative bleeding in patients with acute Stanford type A aortic dissection,and has certain predictive value for the occurrence of severe postoperative bleeding.
