内镜下硬化剂注射与套扎治疗内痔的疗效及复发风险比较

Comparison of the Efficacy and Recurrence Risk of Internal Hemorrhoids Treated by Endoscopic Sclerotherapy Injection and Ligation

目的 比较内镜下硬化剂注射与套扎治疗内痔的疗效及复发风险。方法 选择2021年4月—2022年3月中山市古镇人民医院收治的95例内痔患者为研究对象,采用随机数表法分为硬化剂注射组(n=46)与套扎组(n=49),评估围手术期疼痛评分,评价24 h止血率及3个月内的复发情况。结果 术后1、3 d,硬化剂注射组的疼痛评分明显低于套扎组,差异有统计学意义(P<0.05)。套扎组24 h止血率为95.92%(47/49)明显高于硬化剂注射组的82.61%(38/46),差异有统计学意义(χ^(2)=4.463,P<0.05)。硬化剂注射组复发率6.52%,套扎组复发率为10.20%,两组复发率比较,差异无统计学意义(P>0.05)。结论 内镜下套扎术与硬化剂注射治疗对内痔均有各自的优势,前者止血率更高,而后者术后疼痛更轻,两种方法的复发率相近。

Objective To compare the therapeutic efficacy and recurrence risk of endoscopic sclerotherapy injection and ligation for the treatment of internal hemorrhoids.Methods 95 patients with internal hemorrhoids treated in Guzen People's Hospital of Zhongshan City from April 2021 to March 2022 were selected as the study objects.The pa-tients were divided into serosing agent injection group(n=46)and ligation group(n=49)by random number table method.Perioperative pain scores,24 h hemostasis rate and recurrence within 3 months were evaluated.Results On the 1 and 3 day after operation,the pain score of the sacrosser injection group was significantly lower than that of the ligation group,and the differences were statistically significant(P<0.05).The 24 h hemostasis rate in the ligation group was 95.92%(47/49),which was significantly higher than that in the hardener injection group 82.61%(38/46),and the difference was statistically significant(χ^(2)=4.463,P<0.05).The recurrence rate was 6.52%in the hardener in-jection group and 10.20%in the ligation group.There was no statistically significant difference in the recurrence rate between the two groups(P>0.05).Conclusion Both endoscopic ligation and sclerosing agent injection have their own advantages in the treatment of internal hemorrhoids.The former has a higher hemostasis rate,while the latter has less postoperative pain.The recurrence rates of the two methods are similar.
