米非司酮联合甲氨蝶呤治疗异位妊娠的临床疗效和安全性分析

Clinical Efficacy and Safety Analysis of Mifepristone Combined with Methotrexate in the Treatment of Ectopic Pregnancy

目的 探讨对异位妊娠患者采用米非司酮+甲氨蝶呤完成治疗后获得的临床效果以及安全性。方法 选取2020年2月—2022年7月临沂市河东医院收治的68例异位妊娠患者为研究对象。采用投掷硬币法分为参照组、研究组,各34例。参照组应用甲氨蝶呤治疗;研究组在参照组的基础上,增加米非司酮治疗。对比两组临床疗效。结果 研究组患者治疗总有效率为94.12%,高于参照组,差异有统计学意义(χ^(2)=5.314,P<0.05);研究组患者不良反应(肝功能异常、肠道异常以及口腔溃疡)总发生率低于参照组,差异有统计学意义(P<0.05)。治疗前,两组患者盆腔包块直径以及血人绒毛膜促性腺激素(human chorionic gonadotropin, HCG)水平比较,差异无统计学意义(P>0.05);治疗后,研究组盆腔包块直径以及血HCG水平均低于参照组,差异有统计学意义(P<0.05)。治疗前,两组患者雌二醇水平比较,差异无统计学意义(P>0.05);治疗后,研究组雌二醇水平低于参照组,差异有统计学意义(P<0.05)。结论 临床对异位妊娠患者采用米非司酮+甲氨蝶呤完成治疗后,对于患者药物疗效、安全性提升,盆腔包块直径、血HCG、雌二醇水平的改善,可以获得显著效果。

Objective To investigate the clinical effect and safety of mifepristone plus methotrexate in ectopic preg-nancy.Methods A total of 68 patients with ectopic pregnancy admitted to Hedong Hospital of Linyi City from Febru-ary 2020 to July 2022 were selected as the study objects.They were divided into the reference group and the study group according to the coin toss method,with 34 cases in each group.The patients of the reference group were treated by methotrexate and the patients of the study group was treated by mifepristone based on the reference group.The clinical efficacy between the two groups was compared.Results The total effective rate in the study group was 94.12%,which was higher than that of the reference group,and the difference was statistically significant(χ^(2)=5.341,P<0.05).Compared with the control group,the total incidence rate of adverse reactions(abnormal liver function,intes-tinal abnormalities and oral ulcers)was significantly lower in the study group,and the difference was statistically sig-nificant(P<0.05).Before treatment,the pelvic mass diameter and blood human chorionic gonadotropin(HCG)level be-tween the two groups were compared,and the difference was not statistically significant(P>0.05).After treatment,the pelvic mass diameter and blood HCG level in the study group were lower than that of the reference group,and the dif-ference was statistically significant(P<0.05).Before treatment,the estradiol level between the two groups were com-pared,and the difference was not statistically significant(P>0.05).After treatment,the estradiol level in the study group was lower than that of the reference group,and the difference was statistically significant(P<0.05).Conclusion After the clinical treatment of ectopic pregnancy patients with mifepristone+methotrexate,the drug efficacy and safety of patients can be improved,and the improvement of pelvic mass diameter,blood HCG and estradiol levels can achieve significant results.
