翼状胬肉切除术联合角膜缘干细胞移植术治疗翼状胬肉的效果观察

Effect of Pterygium Excision Combined with Limbal Stem Cell Transplantation on Pterygium

目的:探讨翼状胬肉切除术联合角膜缘干细胞移植术治疗翼状胬肉的效果。方法:抽选取2022年4月—2023年4月贵州水矿控股集团有限责任公司总医院收治的80例翼状胬肉患者作为研究对象,随机分为对照组与研究组,各40例。对照组实施翼状胬肉切除术,研究组在对照组基础上实施角膜缘干细胞移植术。比较两组治疗效果。结果:研究组角膜上皮修复时间、拆线时间短于对照组,裸眼视力高于对照组,复发率低于对照组,差异有统计学意义(P<0.05)。研究组治疗总有效率高于对照组,差异有统计学意义(P=0.013)。术前,两组干眼症状评分(OSDI)、泪膜破裂时间(BUT)、角膜损伤情况评分(FL)比较,差异无统计学意义(P>0.05);术后,两组OSDI、BUT、FL均优于术前,且研究组优于对照组,差异有统计学意义(P<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P=0.013)。结论:翼状胬肉切除术联合角膜缘干细胞移植术治疗翼状胬肉的效果显著,可改善患者泪膜功能及手术指征,降低复发率、并发症发生率。

Objective:To investigate the effect of pterygium excision combined with limbal stem cell transplantation in the treatment of pterygium.Methods:A total of eighty patients with pterygium treated in the General Hospital of Guizhou Water Mining Holding Group Co.,Ltd from April 2022 to April 2023 were selected as the study objects and randomly divided into control group and study group,with forty cases each.Pterygium resection was performed in the control group,and limbal stem cell transplantation was performed in the study group based on the control group.The therapeutic effect of the two groups was compared.Results:The corneal epithelial repair time and stitches removal time in the study group were shorter than those in the control group,the naked eye visual acuity was higher than that in the control group,and the recurrence rate was lower,the difference was statistically significant(P<0.05);The total effective rate of the study group was higher than that of the control group,the difference was statistically significant(P=0.013);Before surgery,there was no significant difference in dry eye symptom score(OSDI),tear film rupture time(BUT)and corneal injury score(FL)between two groups(P>0.05);After surgery,OSDI,BUT and FL in both groups were better than before operation,and the study group was better than the control group,the difference was statistically significant(P<0.05);The complication rate of the study group was lower than that of the control group,and the difference was statistically significant(P=0.013).Conclusion:Pterygium excision combined with limbal stem cell transplantation is effective in the treatment of pterygium,which can improve tear film function and surgical indications,and reduce the recurrence rate and complication rate.
