标准入路联合后内侧双切口入路关节镜下治疗后交叉韧带胫骨止点撕脱骨折的临床疗效分析

Clinical Efficacy of Standard Approach Combined with Posteromedial Double Incision Approach in the Treatment of Posterior Cruciate Ligament Tibial Occlusion Avulsion Fracture under Arthroscopy

目的:分析标准入路联合后内侧双切口入路关节镜下治疗后交叉韧带胫骨止点撕脱骨折的临床疗效。方法:选择2021年6月—2023年1月铜仁市人民医院收治的后交叉韧带胫骨止点撕脱骨折患者30例作为观察组,患者行标准入路联合后内侧双切口入路关节镜下治疗;选择2017年1月—2021年5月该医院收治的后交叉韧带胫骨止点撕脱骨折患者30例作为对照组,患者应用传统切开复位内固定治疗。比较两组患者的术后恢复情况。结果:观察组手术成功率为100%,对照组手术成功率为70.00%(9例患者的治疗效果不理想),观察组手术成功率高于对照组,差异有统计学意义(P<0.05)。治疗前,两组Lysholm膝关节评分、膝关节活动度比较,差异无统计学意义(P>0.05);治疗后,观察组Lysholm膝关节评分、膝关节活动度高于对照组,差异有统计学意义(P<0.05)。结论:在后交叉韧带胫骨止点撕脱骨折患者中行标准入路联合后内侧双切口入路关节镜下治疗,其疗效显著,可有效治疗患者的疾病,促进患者膝关节功能的恢复。

Objective:To analyze the clinical efficacy of standard approach combined with posteromedial double incision approach in the treatment of posterior cruciate ligament avulsion fracture of tibia.Methods:A total of thirty patients with posterior cruciate ligament tibial occlusion avulsion fracture admitted to Tongren People's Hospital from June 2021 to January 2023 were selected as the observation group.The patients underwent arthroscopic treatment through standard approach combined with posteromedial double incision approach.A total of thirty patients with posterior cruciate ligament avulsion fracture of tibia insertion admitted to this hospital from January 2017 to May 2021 were selected as the control group,and the patients were treated with traditional open reduction and internal fixation.The postoperative recovery of the two groups was compared.Results:The success rate of operation in the observation group was 100%,and that in the control group was 70.00%(9 patients had unsatisfactory treatment effect);The success rate of operation in the observation group was higher than that in the control group,with statistical significance(P<0.05);Before treatment,there was no significant difference in Lysholm knee score and knee motion between the two groups(P>0.05);After treatment,Lysholm knee joint score and knee joint motion in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Patients with posterior cruciate ligament tibial occlusion avulsion fracture were treated with standard approach combined with posteromedial double incision approach under arthroscopy,which has significant curative effect and can effectively treat the disease of patients and promote the recovery of knee joint function.
