床旁彩超引导下颈内静脉穿刺在危重症患者抢救治疗中的应用价值分析

Application Value of Internal Jugular Vein Puncture Guided by Bedside Color Ultrasound in Rescue Treatment of Critically Ill Patients

目的:探究在抢救治疗危重症患者时实施床旁彩超引导下颈内静脉穿刺的应用价值。方法:将喀什地区第一人民医院急救中心2022年7月—2023年5月收治的危重症患者40例作为研究对象,使用双色球分组原则将患者分为参照组与试验组,各20例。参照组采用盲穿穿刺置管,试验组采用床旁彩超引导下颈内静脉穿刺置管。对比两组穿刺效果及并发症发生率。结果:试验组患者一次性穿刺成功率高于参照组,差异有统计学意义(P=0.037)。试验组患者穿刺后并发症发生率低于参照组,差异有统计学意义(P<0.05)。试验组患者平均穿刺时间短于参照组,平均静脉置管时间长于参照组,疼痛评分低于参照组,差异有统计学意义(P<0.05)。结论:在危重症患者抢救治疗的过程中采用床旁彩超引导下颈内静脉穿刺置管安全性较高,能减轻反复穿刺给患者带来的痛苦,值得临床应用并予以推广。

Objective:To explore the application value of internal jugular vein puncture guided by color ultrasound in the rescue and treatment of critically ill patients.Methods:Forty critically ill patients admitted to the Emergency Center of Kashgar Region First People's Hospital from July 2022 to May 2023 were taken as the research objects,and the patients were divided into the reference group and the experimental group with twenty cases in each group using the two-color ball grouping principle.The reference group was catheterized by blind puncture,and the experimental group was catheterized by internal jugular vein puncture guided by bedside color ultrasound.The puncture effect and complication rate of the two groups were compared.Results:The success rate of one-time puncture in experimental group was higher than the reference group,the difference was statistically significant(P=0.037);The incidence of postoperative complications in experimental group was lower than the reference group,with statistical significance(P<0.05);The average puncture time of experimental group was shorter than that of the reference group,the average intravenous catheter insertion time was longer than that of the reference group,and the pain score was lower than that of the reference group,the difference was statistically significant(P<0.05).Conclusion:In the process of rescue and treatment of critically ill patients,the use of internal jugular vein puncture guided by bedside color ultrasound has high safety,can reduce the pain caused by repeated puncture,and is worthy of clinical application.
