鼻内镜下中鼻甲处理方式对黏膜功能、疼痛程度、生活质量的影响

The Effect of Nasal Endoscopic Middle-Turbinate Manipulation on Mucosal Function,Pain level and Quality of life

目的:分析鼻内镜下中鼻甲处理方式对黏膜功能、疼痛程度、生活质量的影响。方法:选取2021年1月—2022年12月云浮市新兴县中医院就诊行鼻内镜手术的患者100例为研究对象,根据不同中鼻甲处理方式分为对照组与观察组,各50例。两组患者均行鼻内腔镜手术,对照组术中保留中鼻甲,观察组术中切除中鼻甲。比较两组患者黏膜功能、疼痛程度及生活质量。结果:术后6个月,观察组纤维鼻内镜检查的量化评分、Lund-Mackay评分、T&T标准嗅觉测试法评分低于对照组,差异有统计学意义(P<0.05);出院时、术后6个月,观察组视觉模拟评分法评分低于对照组,差异有统计学意义(P<0.05);出院时、术后6个月,观察组鼻腔鼻窦结局测试-20评分低于对照组,差异有统计学意义(P<0.05)。结论:鼻内镜下切除中鼻甲可有效治疗慢性鼻窦炎,不仅能够改善黏膜功能,且疼痛程度较低,可提高患者的生活质量,值得在临床推广。

Objective:To analyze the effects of endonasal endoscopic middle turbinate manipulation on mucosal function,pain lev⁃el,and quality of life.Methods:100 patients who visited Xinxing County Hospital of Traditional Chinese Medicine in Yunfu City and underwent endonasal endoscopic surgery from January 2021 to December 2022 were selected for the study,and were divided into a control group and an observation group of 50 cases each according to different middle turbinate surgical methods.Both groups of patients underwent nasal endoscopic surgery,the middle turbinate retained in the control group,while the middle turbi⁃nate removed in the observation group.The mucosal function,pain level and quality of life of patients in the two groups were com⁃pared.Results:Six months after the operation,the quantitative score of under fiberoptic nasal endoscopy,Lund-Mackay score,and T&T standard olfactory test score of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05);at the time of hospital discharge and 6 months after the operation,the score of Visual Analog Scale of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05);at the time of discharge and 6 months after the operation,the score of Sino-Nasal Outcome Test-20 in the observation group was low⁃er than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Nasal endoscopic resection of middle turbinate can effectively treat chronic rhinosinusitis,which not only improves the mucosal function,but also has lower pain,which can improve the quality of life of patients,and is worth promoting in the clinic.
