类风湿性关节炎诊断及病情评估中血清类风湿因子联合抗环瓜氨酸肽抗体检测的应用价值

目的:分析类风湿性关节炎(RA)诊断及病情评估中血清类风湿因子(RF)联合抗环瓜氨酸肽(CCP)抗体检测的应用价值。方法:选取2021年4月—2023年2月江门市人民医院收治的疑似RA患者84例为研究对象,均检测血清RF、抗CCP抗体水平。比较RF、抗CCP抗体、RF和抗CCP抗体联合检测结果,RF和抗CCP抗体单一检测和联合检测在RA诊断中的诊断效能。结果:84例疑似RA患者中,抗CCP抗体检测阳性率为60.71%(51/84),RF检测阳性率为54.76%(46/84),RF和抗CCP抗体联合检测阳性率为75.00%(63/84),三种检测方法阳性检出率比较,差异无统计学差异(P>0.05)。RF和抗CCP抗体联合检测灵敏度高于抗CCP抗体、RF检测,差异有统计学意义(P<0.05);RF和抗CCP抗体联合检测准确率高于RF检测,差异有统计学意义(P<0.05)。结论:抗CCP抗体、RF联合检测RA相较于单一检测诊断效能更高,对RA患者预后结局评估有重要价值。

Objective:To analyze the value of serum rheumatoid factor(RF)test combined with anti-cyclic citrullinated peptide(CCP)antibody test in the diagnosis and prognosis assessment of rheumatoid arthritis(RA).Methods:84 patients with suspected RA admitted to Jiangmen People's Hospital from April 2021 to February 2023 were selected as the study subjects,and all of them were tested for serum RF and anti-CCP antibody levels.Results of RF-test,anti-CCP antibody test,and combined RF and anti-CCP anti⁃body test were compared,and then the diagnostic efficacy of single RF and anti-CCP antibody test and combined test in RA diagno⁃sis were analyzed.Results:among 84 patients with suspected RA,the positive rate of anti-CCP antibody test was 60.71%(51/84),the positive rate of RF test was 54.76%(46/84),and the positive rate of combined RF and anti-CCP antibody test was 75.00%(63/84),and the comparison of positive detection rate among the three methods showe dnostatistical difference(P>0.05).the detection sensitivity of combined RF and anti CCP antibody test was higher than that of single anti-CCP antibody and RF test,and the differ⁃ence was statistically significant(P<0.05);the accuracy of combined RF and anti-CCP antibody test was higher than that of single RF test,and the difference was statistically significant(P<0.05).Conclusion:The combination of anti-CCP antibody test and RF test is more effective in the diagnosis of RA than the single test,and it is valuable in the assessment of the prognosis of RA patients.
