摘要
目的:探析动态心电图(DCG)对冠心病心肌缺血(CHD-MI)的诊断价值。方法:选取2020年5月—2023年4月临沂市平邑县人民医院收治的疑似CHD-MI患者71例为研究对象,均接受常规心电图(ECG)、DCG检查。比较两种检查方式阳性检出率及诊断效能,并绘制工作特征曲线,运用曲线下面积分析ECG、DCG两种检查方式诊断价值。结果:CAG检查后,46例(64.79%)确诊为CHD-MI患者;ECG检查后,44例(61.97%)为阳性,27例(38.03%)为阴性;DCG检查后,46例(64.79%)为阳性,25例(35.21%)为阴性。DCG检查敏感性、特异性、准确性、阳性预测值及阴性预测值均高于ECG检查,差异有统计学意义(P<0.05)。DCG检查曲线下面积大于ECG检查(P<0.05)。结论:DCG检查在CHD-MI患者疾病诊断方面具有较高价值,阳性检出率及诊断效能较高,有助于提升CHD-MI的诊断效率。
Objective:to explore the diagnostic value of dynamic electrocardiogram(DCG)on coronary heart disease-myocardial ischemia(CHD-MI).Methods:71 patients with suspected CHD-MI admitted to the People's Hospital of Pingyi County,Linyi City,from May 2020 to April 2023 were selected as study subjects,and all of them underwent routine electrocardiogram(ECG)and DCG examinations.The positive detection rate and diagnostic efficacy of the two examinations were compared,and the work⁃ing characteristic curve was drawn to analyze the diagnostic value of ECG and DCG using the area under the curve.Results:After CAG examination,46 cases(64.79%)were diagnosed as CHD-MI patients;after ECG examination,44 cases(61.97%)were posi⁃tive and 27 cases(38.03%)were negative;after DCG examination,46 cases(64.79%)were positive and 25 cases(35.21%)were negative.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of DCG were higher than those of ECG,and the difference was statistically significant(P<0.05).The area under the curve of DCG examination was larger than that of ECG examination(P<0.05).Conclusion:DCG examination has high value in the diagnosis of CHD-MI patients,with high positive detection rate and diagnostic efficacy,which helps to improve the diagnostic efficiency of CHD-MI.
