血清胃泌素-17、胃蛋白酶原联合幽门螺杆菌在早期胃癌筛查中的应用价值

Application Value Analysis of Serum Gastrin-17,Pepsinogen and Helicobacter Pylori in Early Gastric Cancer Screening

目的:探讨血清胃泌素-17(G-17)、胃蛋白酶原(PG)联合幽门螺杆菌(Hp)在早期胃癌筛查中的应用价值。方法:选取2021年6月—2023年2月单县中心医院收治的胃部疾病患者117例作为观察对象,按照内镜检查及病理组织学检查结果分为良性病变(n=59)、癌前病变(n=34)、早期胃癌(n=24)。检查患者G-17、PG、Hp情况,比较各组患者G-17、PGⅠ、PGⅡ、PGR水平及Hp阳性率,分析各指标诊断早期胃癌的价值。结果:早期胃癌、癌前病变患者G-17、PGⅠ、PGⅡ水平及Hp阳性率高于良性病变患者,PGR低于良性病变患者,早期胃癌患者G-17、PGⅡ水平高于癌前病变患者,差异有统计学意义(P<0.05)。G-17、PGR、Hp阳性的受试者工作曲线下面积(AUC)分别为0.695、0.765、0.815,联合检测AUC为0.902。结论:G-17、PGⅠ、PGⅡ、Hp指标与胃癌的发生、进展有关,进行多指标联合检测在早期胃癌诊断中具有较高价值。

Objective:To explore the application value of serum gastrin-17(G-17),pepsinogen(PG)and helicobacter pylori(Hp)in early gastric cancer screening.Methods:A total of 117 patients with gastric diseases admitted to Shanxian Central Hospital from June 2021 to February 2023 were selected as the observation objects.According to the results of endoscopic and histopathologic examinations,they were divided into benign lesions(n=59),precancerous lesions(n=34),and early gastric cancer(n=24).The G-17,PG,and Hp of patients were checked.The levels of G-17,PGⅠ,PGⅡ,PGR,as well as Hp positive rate were compared among different groups of patients,and the correlation between indicators and gastric cancer,as well as the diagnostic value of early gastric cancer were analyzed.Results:The levels of G-17,PGⅠ,PGⅡ,as well as Hp positive rate in patients with early gastric cancer and precancerous lesions were higher than those in patients with benign lesions,while the PGR in patients with early gastric cancer and precancerous lesions was lower than that in patients with benign lesions.The levels of G-17 and PGⅡin patients with early gastric cancer were higher than those in patients with precancerous lesions,and the difference was statistically significant(P<0.05).The area under the receiver operating characteristic curve(AUC)for G-17,PGR,and Hp positivity was 0.695,0.765,and 0.815,while the combined detection AUC was 0.902.Conclusion:The G-17,PGⅠ,PGⅡ,and Hp indicators are related to the occurrence and progression of gastric cancer.The combined detection of multiple indicators can accurately detect gastric cancer in the early stage.
