γ干扰素释放试验在结核病与糖尿病共病中的诊断价值研究

Diagnostic Value ofγInterferon Release Test in Tuberculosis and Diabetes Comorbidity

目的:探讨γ干扰素(IFN-γ)释放试验在结核病与糖尿病共病中的诊断价值。方法:选取2021年3月—2022年11月中山市第二人民医院感染科收治的初治肺结核患者92例为研究对象,2021年3月—2021年8月采用结核感染T细胞斑点试验(T-SPOT.TB)试剂盒检测,2021年9月—2022年11月采用IFN-γ试剂盒(TB-IGRA)检测及糖化血红蛋白(HBA1c)检测。根据HBA1c范围分为A组(4%~6.5%),B组(6.6%~9%),C组(>9%)。分析T-SPOT.TB、TB-IGRA、HBA1c检测结果及T-SPOT.TB、TB-IGRA检测肺结核阳性在各组中占比。结果:HBA1c检测结果,A组27例,B组34例,C组31例;TB-IGRA检测阳性率高于T-SPOT.TB检测阳性率,差异有统计学意义(P=0.024)。A组、B组、C组TB-IGRA检测阳性占比均高于T-SPOT.TB检测阳性占比,差异有统计学意义(P<0.05)。结论:TB-IGRA具有较高的检出率,对诊断肺结核合并糖尿病患者的阳性率影响较小,利于及时控制肺结核的传播。

Objective:To investigate the diagnostic value ofγinterferon(IFN-γ)release test in tuberculosis and diabetes comorbidity.Methods:A total of 92 patients with primary tuberculosis who were admitted to Infection Department of Zhongshan Second People's Hospital from March 2021 to November 2022 were selected as the study subjects.From March 2021 to August 2021,the tuberculosis infection T cell spot test(T-SPOT.TB)kit detection was used.From September 2021 to November 2022,IFN-γkit(TB-IGRA)and glycosylated hemoglobin(HBA1c)detection were used.According to the HBA1c range,the patients were divided into group A(4%-6.5%),group B(6.6%-9%),and group C(>9%).The results of T-SPOT.TB,TB-IGRA and HBA1cdetection and the proportion of positive tuberculosis detected by T-SPOT.TB and TB-IGRA in each group were analyzed.Results:HBA1c detection results showed 27 cases in group A,34 cases in group B,and 31 cases in group C.The positive rate of TB-IGRA detection was higher than that of T-SPOT.TB detection,and the difference was statistically significant(P=0.024).The proportion of positive by TB-IGRA detection in group A,group B and group C was higher than that of T-SPOT.TB detection,and the difference was statistically significant(P<0.05).Conclusion:TB-IGRA has high diagnostic accuracy and little influence on the positive rate of patients with tuberculosis and diabetes,and is conducive to timely control the transmission of tuberculosis.
