居家养老健康管理模式在社区养老服务中的应用效果

Application Effect of Home-Based Elderly Care Health Management Model in Community Elderly Care Service

目的:探讨居家养老健康管理模式在社区居家养老服务中的实践效果。方法:选取2021年7月—2022年5月北京市昌平区沙河医院收治的患者200例为研究对象,根据随机数字表法分为对照组和研究组,各100例。对照组实施常规家庭医生签约管理,研究组实施居家养老健康管理模式。比较两组患者短期身体状况、心理状况、睡眠质量及生活能力、原发慢性疾病急性发作情况。结果:研究组短期身体状况优良率高于对照组,差异有统计学意义(P<0.001);干预后,两组焦虑、抑郁、睡眠质量评分均降低,生活能力评分均升高,研究组优于对照组,差异有统计学意义(P<0.05);研究组原发慢性疾病急性发生率低于对照组,差异有统计学意义(P<0.001)。结论:居家养老健康管理模式在社区养老服务中的实践效果良好,有利于改善患者负性情绪、睡眠质量及生活能力,减少原发慢性疾病发作情况。

Objective:To explore the practice effect of home-based elderly care health management model in community home-based elderly care service.Methods:A total of 200 patients who were admitted to Beijing Changping District Shahe Hospital from July 2021 to May 2022 were selected as the study subjects.They were divided into the control group and the study group according to random number table method,with 100 cases in each group.The control group implemented conventional family doctor contracted management,and the study group implemented home-based elderly care health management model.The short-term physical condition,psychological condition,sleep quality,living ability,and acute episode of primary chronic disease in two groups were compared.Results:The excellent rate of short-term physical condition in the study group was higher than that in the control group,and the difference was statistically significant(P<0.001);after intervention,the scores of anxiety,depression and sleep quality were decreased in both groups,while the scores of living ability were increased in both groups,the study group was better than the control group,and the difference was statistically significant(P<0.05);the acute incidence of primary chronic diseases in the study group was lower than that in the control group,and the difference was statistically significant(P<0.001).Conclusion:The practice effect of the home-based elderly care health management model in community elderly care services is good,which is conducive to improving patients'negative emotions,sleep quality and living ability,reducing the onset of primary chronic diseases.It can provide community health service agencies with a model of"integrated medical care and elderly care"health management services.
