磁共振弥散峰度成像鉴别诊断前列腺癌与良性前列腺增生的临床价值及参数分析

Clinical value and parameter analysis of magnetic resonance diffusion kurtosis imaging in differential diagnosis of prostate cancer and benign prostatic hyperplasia

目的:探析磁共振弥散峰度成像(DKI)在鉴别诊断前列腺癌与良性前列腺增生中的应用价值,并分析相关参数。方法:选取聊城市茌平区第二人民医院2022年1月—2023年1月收治的前列腺疾病患者90例,均接受DKI检查及组织病理学检查,分析DKI在鉴别诊断前列腺癌与良性前列腺增生中的临床价值。结果:以组织病理检查结果作为诊断金标准,90例患者中共检出前列腺癌41例,良性前列腺增生49例。DKI鉴别诊断前列腺癌及良性前列腺增生的准确率为88.89%,灵敏度为85.37%,特异度为91.84%,DKI评估诊断前列腺癌、良性前列腺增生患者结果具有极好的一致性(Kappa值=0.775);前列腺癌患者平均扩散系数(MD)值低于良性前列腺增生患者,平均扩散峰度(MK)、轴向峰度(Ka)、径向峰度(Kr)和表观弥散系数(ADC)均高于良性前列腺增生患者(P <0.05)。结论:前列腺癌与良性前列腺增生鉴别诊断采用DKI的临床价值较高。

Objective To explore the application value of magnetic resonance diffusion kurtosis imaging(DKI)in the differential diagnosis of prostate cancer and benign prostatic hyperplasia(BPH),and analyze relevant parameters.Methods A total of 90 patients with prostate disease admitted to Liaocheng Chiping District Second People's Hospital from January 2022 to January 2023 were selected,all of whom underwent magnetic resonance DKI and histopathological examination to analyze the clinical value of magnetic resonance DKI in the differential diagnosis of prostate cancer and BPH.Results Using histopathological examination results as the gold standard for diagnosis,a total of 41 cases of prostate cancer were detected in 90 patients,49 cases of BPH.The accuracy,sensitivity and specificity of differential diagnosis of prostate cancer and BPH by DKI was 88.89%,85.37%and 91.84%.The results of DKI in the diagnosis of prostate cancer and BPH showed excellent consistency(Kappa value=0.775).The mean diffusion coefficient(MD)of patients with prostatic carcinoma was lower than that of patients with BPH,and the mean diffusion kurtosis(MK),axial kurtosis(Ka),radial kurtosis(Kr)and apparent diffusion coefficient(ADC)were higher than those of patients with BPH(P<0.05).Conclusion The differential diagnosis between prostate cancer and benign prostatic hyperplasia using magnetic resonance diffusion kurtosis imaging has high clinical value.
