张念志教授从“痰热瘀虚”论治支气管扩张症经验

支气管扩张症是一种呼吸系统常见病,易反复感染发作使支气管异常和持久扩张,不可逆转,需长期治疗。西医治疗以抗感染为主,但耐药问题日渐突出,疗效欠佳,而中医药治疗具有自身优势。张念志教授认为支气管扩张症病程漫长,多虚实夹杂,以痰、热、瘀为主要病机,贯穿疾病全程,日久耗伤正气,以致肺脾气虚、肺肾阴虚。急性期治疗以凉血止血、泻火化痰、散瘀活血为主,缓解期以补益肺脾、养阴益肾为要。张教授临证权衡标本缓急,辨证论治,灵活化裁,疗效显著。

Bronchiectasis is a common disease of the respiratory system,and is prone to repeated infection.The abnormal bronchus and persistent bronchiectasis are irreversible and require long-term treatment.Western medicine treatment is mainly based on anti-infection,but the problem of drug resistance is becoming more and more prominent,and the curative effect is not good,while traditional Chinese medicine treatment has its own advantages.Professor Zhang Nianzhi believes that the course of bronchiectasis is long,and bronchiectasis is characterized by the combination of deficiency and excess.The phlegm,heat and blood stasis are the main pathogenesis,and they run through the whole course of the disease.For a long time,healthy Qi is consumed,resulting in Qi deficiency of lung and spleen and Yin deficiency of lung and kidney.The treatments in acute stage are mainly to cool blood and stop bleeding,clear away fire and resolve phlegm,disperse stasis and promote blood circulation,and the treatments in the remission stage are to tonify lung and spleen,nourish Yin and tonify kidney.In clinical practice,Prof.Zhang weighs the urgency of root cause and manifestation,treats the disease based on syndrome differentiation,and modifies the formula flexibly,with remarkable curative effect.
