基于二元疾病管理理论在老年脑卒中恢复期患者出院准备服务方案中的应用

Practice of binary disease management theory in the discharge rehabilitation service program of elderly convalescent stroke patients

目的探讨基于二元疾病管理理论在老年脑卒中恢复期患者出院准备服务方案中的应用效果。方法采用便利抽样法,选取本院2021年5月—2022年6月收治的96例老年脑卒中患者作为研究对象,按照患者入院的先后顺序随机分为对照组(48例)与观察组(48例);对照组给予常规护理措施,观察组实施基于二元疾病管理理论的出院准备服务方案,采用中文版医院出院准备度量表评估患者的出院准备度状况、采用汉化的二元应对量表评估患者及照顾者的二元疾病应对情况、采用癌症照顾者综合需求量表评估照顾者照顾需求情况。结果实施基于二元疾病管理理论以后,观察组患者的出院准备度在个人状况、适应能力、预期性支持方面的得分高于对照组,差异有统计学意义(P<0.001);观察组患者和照顾者的二元疾病应对得分高于对照组,差异有统计学意义(P<0.001);观察组照顾者的照顾需求总分低于对照组(P<0.001)。结论对老年脑卒中恢复期患者实施基于二元疾病管理理论的出院准备服务方案,能够提高患者的出院准备度及支持水平,满足照顾者的需求。

Objective To explore the effect of binary disease management theory in elderly convalescent stroke patients.Methods In this study,using the convenience sampling method,this study selected 96 elderly stroke patients admitted to a tertiary hospital from May 2021 to June 2022,randomly divided them into control group(48 cases)and observation group(48 cases),the control group gave routine care measures,adopted the discharge preparation service plan of binary disease management theory,and conducted questionnaire survey to patients with the discharge readiness scale,binary disease response scale and caregiver comprehensive demand table.Results After the implementation of the binary disease management theory,patients in the observation group had better scores in personal status,adaptability,anticipatory support than the control group,the difference was statistically significant(P<0.001);The binary disease response scores of the patients and caregivers in the observation group were better than that of the control group,the difference was statistically significant(P<0.001);The total care score of caregivers in the observation group was lower than that in the control group(P<0.001).Conclusion The discharge preparation service plan of binary disease management theory for elderly stroke patients in the recovery period can improve the discharge readiness and support level of patients and meet the needs of caregivers.
