摘要
[目的]探讨Ⅱ型糖尿病骨密度与认知障碍的相关性。[方法]2020年7月—2021年9月45例Ⅱ型糖尿病(type II diabetes mellitus,T2DM)患者纳入本研究。检测血糖,糖化血红蛋白(HbA1c)、血清骨钙素(osteocalcin,OC),检测骨密度(bone mineral density,BMD),采用蒙特利尔认知评估表(the Montreal Cognitive Assessment,MoCA)评估患者的认知水平。[结果]按BMD检测结果,16例为骨质疏松,17例为骨量减少,12例为骨量正常。认知障碍发生,骨质疏松组为15/16(93.8%);骨量减低组为13/17(76.5%),骨质正常组为3/12(25.0%)。按是否发生认知障碍将患者分为两组,两组患者在年龄、性别构成、体重、身高、BMI、HbA1c以及受教育年限的差异均无统计学意义(P>0.05)。认知障碍组患者的MoCA评分、OC以及BMD显著低于认知正常组(P<0.05)。[结论]T2DM患者随BMD下降认知障碍发生率明显增加。
[Objective]To investigate the relationship between bone mineral density and cognitive impairment in type Ⅱ diabetes mellitus.[Methods]From July 2020 to September 2021,45 patients with type Ⅱ diabetes mellitus(T2DM)were enrolled in this study.Blood glucose,glycated hemoglobin Alc(HbAlc),serum osteocalcin(OC)and bone mineral density(BMD)were detected,while the Montreal Cognitive Assessment(MoCA)was used to evaluate the cognitive level of the patients.[Results]According to the results of BMD,16 cases were osteoporosis,17 cases were bone mass reduction,and 12 cases were normal bone mass.The incidence of cognitive impairment was 15/16(93.8%)in the osteoporosis group,whereas 13/17(76.5%)in the bone mass reduction group,and 3/12(25.0%)in the normal bone mass group.The patients were divided into two groups according to whether they had cognitive impairment.There were no significant differences in age,gender composition,weight,height,BMI,HbAlc and years of education between the two groups(P<0.05),the cognitive impairment group had significantly lower MoCA score,OC and BMD than the normal cognition group(P<0.05).[Conclusion]The incidence of cognitive impairment is significantly increased with the decrease of BMD in T2DM.
