耳穴压豆联合中药足浴对剖宫产术后快速康复的影响

目的探讨耳穴压豆联合穴位按摩、中药足浴对剖宫产产后泌乳、乳房胀痛及快速康复的影响。方法选择2019年6月至2021年2月本院经剖宫产分娩的产妇214例,按1∶1比例随机分为对照组与观察组。对照组107例实施穴位按摩、中药足浴干预,观察组107例在对照组基础上进行耳穴压豆干预,两组均干预5 d。观察两组产妇泌乳始动时间,统计两组干预1 d、5 d后泌乳量,评定两组乳房胀痛情况,检测两组干预前、干预5 d后血清泌乳素(PRL)及血清炎症因子[白细胞介素-6(IL-6)、C-反应蛋白(CRP)]水平,并记录两组肛门排气时间、离床时间及干预5 d后宫底高度。结果观察组产妇泌乳始动时间明显较对照组短(P<0.05),干预1 d、5 d后泌乳量均明比较对照组多(P<0.05);干预后两组血清PRL水平与干预前比明显增高(P<0.05),且观察组干预后比对照组高(P<0.05);观察组乳房肿痛程度比对照组轻(P<0.05);干预后两组血清IL-6、CRP水平与干预前比均明显降低(P<0.05),且观察组均比对照组低(P<0.05);观察组肛门排气时间、离床时间比对照组短(P<0.05),干预5 d后宫底高度比对照组低(P<0.05)。结论剖宫产产后给予耳穴压豆联合穴位按摩、中药足浴干预,可明显上调血清PRL水平及下调血清IL-6、CRP水平,有效促进泌乳,提高泌乳量,并能减轻乳房肿痛程度,促使产妇康复进程加快。

Objective To explore the effects of ear acupoint pressing bean combined with artificial acupoint stimulation and traditional Chinese medicine foot bath on lactation,breast swelling pain and rapid recovery after cesarean section.Methods A total of 214 parturients who came to our hospital for cesarean delivery from June 2019 to February 2021,and randomly divided them into the control group and the experimental group at a ratio of 1∶1.The 107 patients in the control group received artificial acupoint stimulation and traditional Chinese medicine foot bath intervention,and 107 patients in the experimental group received ear acupoint pressing bean intervention on the basis of the control group.Both groups were treated for 5 days.The lactation initiation time of the two groups was compared,the amount of lactation in the two groups after 1d and 5d interventions was counted,the breast swelling pain in the two groups was assessed,the serum prolactin(PRL)and serum inflammatory factors[interleukin-6(IL-6),C-reactive protein(CRP)]levels before and 5 days after intervention in the two groups were detected,the anal exhaust time,the time to get out of bed,and the height of the fundus of the uterus after 5 days of intervention were recorded.Results The lactation initiation time of the parturients in the experimental group was significantly earlier than that in the control group(P<0.05),and the lactation volume after intervention for 1 and 5 days was significantly higher than that in the control group(P<0.05).After the intervention,the serum PRL level in the two groups was significantly higher than that before the intervention in the same group(P<0.05),and the experimental group was significantly higher than the control group after the intervention(P<0.05).The degree of breast swelling pain in the experimental group was significantly less than that in the control group(P<0.05).After the intervention,the levels of serum IL-6 and CRP in the two groups were significantly lower than those before the intervention in the same group(P<0.05),and the experimental group were significantly lower than the control group(P<0.05).The anal exhaust time and time to get out of bed in the experimental group were significantly shorter than those in the control group(P<0.05),and the height of the fundus of the uterus was significantly lower than that in the control group after 5 days of intervention(P<0.05).Conclusion After cesarean section,giving ear acupoint pressing bean combined with artificial acupoint stimulation and traditional Chinese medicine foot bath intervention can significantly increase serum PRL level and decrease serum IL-6 and CRP levels,effectively promote lactation,significantly increase lactation,and greatly reduce the degree of breast swelling pain and accelerate the recovery process of the parturients.
