射频消融术治疗儿童慢性鼻窦炎伴腺样体肥大的临床分析

Clinical Analysis of Radiofrequency Ablation for the Treatment of Chronic Rhinosinusitis with Adenoid Hypertrophy in Children

目的 探讨与分析射频消融术治疗儿童慢性鼻窦炎(chronic rhinosinusitis, CRS)伴腺样体肥大的临床效果。方法 随机选取2020年1月—2023年4月宁夏医学大学总医院耳鼻咽喉头颈外科诊治的300例慢性鼻窦炎伴腺样体肥大患儿作为研究对象。采取随机数字表法分为射频组与对照组,每组150例。对照组给予腺样体切除手术治疗,射频组给予射频消融术治疗。比较两组患儿的围手术期指标、总体疗效以及鼻腔结构状况。结果 射频组的住院时间、术中出血量显著少于对照组,差异有统计学意义(P<0.05)。射频组术后14 d的治疗总有效率为98.00%明显高于对照组,差异有统计学意义(χ^(2)=11.521,P<0.05)。术后14 d,两组患儿鼻腔容积、鼻腔最小横截面积均升高,且射频组高于对照组,差异有统计学意义(P<0.05)。结论 射频消融术治疗儿童慢性鼻窦炎伴腺样体肥大并不会增加手术的复杂度,还可减少住院时间、术中出血量,提高对患儿的总体治疗效果。

Objective To investigate and analyze the clinical effect of radiofrequency ablation for the treatment of chronic rhinosinusitis(CRS)with adenoid hypertrophy in children.Methods A total of 300 children with chronic rhi-nosinusitis with adenoid hypertrophy were randomly selected from the Department of Otorhinolaryngology,Head and Neck Surgery of General Hospital of Ningxia Medical University from January 2020 to April 2023 as the study subjects.Random number table method was used to divide the children into RF group and control group,150 cases in each group.The control group was treated with adenoidectomy and the radiofrequency group was treated with radiofrequency ablation.The perioperative indexes,overall efficacy and nasal structure conditions of the two groups were compared.Results The hospitalization time and intraoperative bleeding in the radiofrequency group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).The total effective rate of the radiofre-quency group at 14 days after operation was 98.00%,which was significantly higher than that of the control group,and the difference was statistically significant(χ^(2)=11.521,P<0.05).14 days after surgery,nasal volume and minimum na-sal cross-sectional area were increased in both groups,and the radiofrequency group was higher than the control group,the differences were statistically significant(P<0.05).Conclusion Radiofrequency ablation for the treatment of chronic rhinosinusitis with adenoid hypertrophy in children does not increase the complexity of surgery,but also re-duces hospitalization time,intraoperative bleeding,and improves the overall therapeutic effect on children.
