中国肾病综合征患者CYP3A5基因的遗传平衡规律及多态性对FK506血药浓度影响研究

摘要 目的:探讨中国肾病综合征患者CYP3A5基因遗传平衡规律,以及CYP3A5基因多态性对他克莫司(FK506)血药浓度的影响。方法:纳入肾病综合征患者23例,通过基因多态性检测,推演遗传平衡规律,以FK506进行治疗,并进行药物浓度监测,统计分析不同CYP3A5基因型下FK506血药浓度差异。结果:CYP3A5基因型实际频数与理论频数之间无统计学差异(0.98,P>0.05),符合Hardy-Weinberg平衡定律;不同基因型CYP3A5*3/*3、CYP3A5*3/*1与CYP3A5*1/*1组间,FK506原始谷浓度和标准化浓度均无统计学意义(P>0.05)。结论:中国肾病综合征患者中CYP3A5基因型遗传平衡,本研究中其不同基因型间FK506血药浓度无显著性差异。 Objective:To investigate the genetic balance pattern of CYP3A5 gene and the effect of CYP3A5 gene polymorphism on tacrolimus(FK506)blood concentration in Chinese patients with nephrotic syndrome.Method:Twentythree patients with nephrotic syndrome were included and treated with FK506 by genetic polymorphism detection to deduce the genetic balance pattern.The drug concentration monitoring was performed to statistically analyze the difference of FK506 blood concentration under different CYP3A5 genotypes.Result:There was no statistical difference between the actual and theoretical frequencies of CYP3A5 genotypes(0.98,P>0.05),in accordance with the Hardy-Weinberg equilibrium law.There was no statistically significant difference between the FK506 raw and standardized valley concentrations between the CYP3A5*3/*3,CYP3A5*3/*1 and CYP3A5*1/*1 groups for different genotypes(P>0.05).Conclusion:The CYP3A5 genotype is genetically balanced in Chinese patients with nephrotic syndrome,and there is no significant difference in FK506 blood concentrations between the genotypes in this study.
