肌骨超声在急性闭合性运动损伤诊断中的价值 被引量:1

Value and accuracy of musculoskeletal ultrasound in the diagnosis of acute closed sports injury

摘要 目的探讨急性闭合性运动损伤应用肌骨超声(MSUS)检查的诊断价值。方法回顾性分析2022年1月至2023年5月广州中兴运动损伤专科医院收治的38例疑似急性闭合性运动损伤患者的临床资料,均接受X线、MSUS和磁共振成像(MRI)技术检查,以MRI检查作为诊断“金标准”,比较X线、MSUS检查对急性闭合性运动损伤的检出阳性率及诊断价值。结果38例患者MRI检查均为阳性;X线阳性率为52%;MSUS检查阳性率92%;两种阳性检出率对比,差异有统计学意义(P<0.05);MRI检查显示:肌肉损伤轻度9例、中度1例,韧带挫伤18例、部分断裂3例、完全断裂4例、骨与软骨损伤3例;MSUS对肌肉损伤、韧带损伤、骨与软骨损伤的检出率高于X线检查,其中对轻度肌肉损伤、韧带完全断裂两种检查方式的检出率差异有统计学意义(P<0.05)。结论实施MSUS检查对急性闭合性运动损伤的诊断准确率较高,可实时、动态观察,清晰显示损伤部位的范围、程度、血肿等信息,为医师制定治疗或手术方案提供可靠的参考。 Objective To explore the diagnostic value of applying musculoskeletalultrasound(MSUS) examination for acute closed sports injury.Methods A retrospective analysis was conducted on the clinical data of 38 suspected patients with acute closed sports injuries admitted to Guangzhou Zhongxing Sports Injury Specialist Hospital from January 2022 to May 2023.All patients underwent X-ray,MSUS,and magnetic resonance imaging(MRI)examinations,using MRI as a diagnostic"gold standard",so as to comparatively analyse the detection positive rate of X-ray and MSUS and the diagnostic value of acute closed sports injuries.Results All38 patients showed positive MRI examination;X-ray examination results:the positive rate is 52%;MSUS test results:positive rate 92%;the difference between the two positive detection rates was statistically significant(P<0.05);MRI examination showed that there were 9 cases of mild muscle injury,and 1 case of moderate muscle injury,18 cases of ligament contusion,3 cases of partial rupture,4 cases of complete rupture,and 3 cases of bone and cartilage injury;the detection rate of MSUS for muscle injury,ligament injury,and bone and cartilage injury was higher than that of X-ray examination.There is a significant difference in the detection rate of mild muscle injury and complete ligament rupture between the two examination methods(P<0.05).Conclusion The implementation of MSUS examination has significant diagnostic value for acute closed motion injury,with high diagnostic accuracy,which can be observed in multiple sections,multiple angles,in real time and dynamically,and clearly show the scope,degree,haematoma,and other information of the injury site,and provide a reliable reference for the later physicians to formulate the surgical plans in the later stage,in order to achieve the goal of improving patient's prognosis.
