摘要
目的探究老年高血压病合并糖尿病患者糖脂代谢变化及与肝功能的关系。方法选择2019年1月至2022年12月于江西省赣州市南康区第一人民医院82例患者,根据是否高血压病合并糖尿病分为高血压组(n=27)、合并糖尿病组(n=27)和健康组(n=28)。收集患者的一般资料,检测患者血糖、血脂水平并分析其与肝功能的相关关系。结果合并糖尿病组的体质量指数(BMI)高于高血压病组与健康组(P<0.05),高血压病组的BMI高于健康组(P<0.05);合并糖尿病组空腹血糖(FPG)、餐后2 h血糖(2 h PPG)、糖化血红蛋白(HbA1c)水平高于高血压病组与健康组(P<0.05),高血压病组FPG、2 h PPG、HbA1c水平与健康组比较差异无统计学意义(P>0.05);合并糖尿病组的低密度脂蛋白胆固醇(LDL-C)、三酰甘油(TG)、血清总胆固醇(TC)水平高于高血压病组和健康组(P<0.05),高密度脂蛋白胆固醇(HDL-C)水平低于高血压病组和健康组(P<0.05),高血压病组的LDL-C、TG、TC水平高于健康组(P<0.05),HDL-C水平低于健康组(P<0.05);合并糖尿病组的丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、γ-谷氨酰转移酶(γ-GT)、总胆红素(TB)水平高于高血压病组和健康组(P<0.05),高血压病组的ALT、AST、γ-GT、TB水平高于健康组(P<0.05);相关分析显示,FPG、2 h PPG、HbA1c、LDL-C、TG、TC水平与肝功能异常呈正相关,HDL-C与肝功能异常情况呈负相关。结论老年高血压病合并糖尿病患者肝功能异常与糖脂代谢水平密切相关。
Objective To explore the changes of glucose-lipid metabolism and its relationship with liver function in elderly patients with hypertension combined with diabetes mellitus.Methods A total of 82 patients in First People's Hospital of Nankang District were enrolled as the research objects from January 2019 to December 2022,which were divided into hypertension group(n=27),the combined diabetes group(n=27)and healthy group(n=28)according to whether they were hypertensive combined with diabetes.The general data of patients were collected,the levels of blood glucose and blood lipid were detected,and their correlation with liver function was analyzed.Results The BMI of the combined diabetes group was the higher than that of the hypertensive group and the healthy group(P<0.05),and the BMI of the hypertensive group was higher than that of the healthy group(P<0.05).The levels of fasting blood glucose(FPG),2 h postprandial blood glucose(2 h PPG)and glycosylated hemoglobin(HbA,c)in diabetes group were no significant difference compared with the healthy group(P>0.05).The levels of low density lipoprotein cholesterol(LDL-C),triglyceride(TG)and total cholesterol(TC)were higher in the combined diabetes group than in the hypertensive and healthy groups(P<0.05),while the level of high density lipoprotein cholesterol(HDL-C)were lower than in the hypertensive and healthy groups(P<0.05),and the levels of LDL-C,TG,and TC were higher in the hypertensive group than in the healthy group(P<0.05),and HDLC levels were lower than in the healthy group(P<0.05).The levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),-glutamyltransferase(-GT)and total bilirubin(TB)were higher in the combined diabetes group than in the hypertensive group and the healthy group(P<0.05),and the levels of ALT,AST,-GT,and TB in the hypertensive group were higher than those in the healthy group(P<0.05).Correlation analysis showed that levels of FPG,2 h PPG,HbAlc,LDL-C,TG and TC were positively correlated with abnormal liver function,while HDL-C was negatively correlated with it.ConclusionAbnormal liver function is closely related to glucose-lipid metabolism level in elderly patients with hypertension and diabetes mellitus.
