慢性子宫内膜炎患者内膜中CD38、CD138表达的临床意义及对不孕症的影响

The clinical significance of CD38 and CD138 expression in the endometrium of patients with chronic endometritis and its impact on infertility

目的探讨慢性子宫内膜炎(CE)患者内膜中CD38、CD138表达的临床意义及对不孕症的影响。方法选择宣城市人民医院131例CE患者为研究对象。根据是否合并不孕症,将患者分为CE组(n=63)和CE合并不孕症组(CE+IN组,n=68)。对患者外周血相关激素水平及子宫内膜CD38、CD138的表达情况进行检测,统计分析CD38、CD138表达不孕症发生风险的影响。结果CD38在CE组、CE+IN组患者子宫内膜中的阳性率分别为19.0%(12/63)、52.9%(38/68),CD138的阳性率分别为22.2%(14/63)、57.4%(39/68)。在CE组及CE+IN组患者中,CD38、CD138阳性患者的雌二醇(E2)水平显著高于阴性表达患者(P<0.05);但CD38、CD138的表达对黄体生成素、泌乳素、卵泡刺激素、睾酮及胰岛素等激素的水平无明显影响(P>0.05)。Logistic多因素分析显示,合并子宫肌瘤、内膜CD38阳性、CD138阳性是影响CE患者并发不孕症的独立性危险因素(P<0.05)。结论CE合并不孕症患者内膜组织中CD38、CD138的表达水平显著较高,且CD38、CD138高表达与不孕症的发生风险密切相关。

Objective To investigate the clinical significance of CD38 and CD138 expression in the endometrium of CE patients and their impact on infertility.Methods 131 CE patients in Xuancheng People's Hospital were selected as the study objects.Patients were divided into CE group(n=63)and CE group with infertility(CE+IN group,n=68)according to whether they had infertility.Detect the levels of peripheral blood related hormones and the expression of CD38 and CD138 in the endometrium of patients,and analyze the impact of CD38 and CD138 expression on patient hormone levels and the risk of infertility.Results The positive rates of CD38 in the endometrium of CE group and CE+IN group were 19.0%(12/63)and 52.9%(38/68),respectively,and the positive rates of CD138 were 22.2%(14/63)and 57.4%(39/68),respectively.In the CE group and CE+IN group,E2 levels in patients with positive expression of CD38 and CD138 were significantly higher than those in patients with negative expression(P<0.05).However,the expression of CD38 and CD138 did not significantly affect the levels of LH,PRL,FSH,T,INS and other hormones(P>0.05).Merging uterine fibroids,endometrial CD38 positive,and CD138 positive are independent risk factors for infertility in CE patients(P<0.05).Conclusion The expression levels of CD38and CD138 in the endometrium of CE patients with infertility are significantly higher,and the high expression of CD38 and CD138 is closely related to the risk of infertility.
