血清MIF水平检测及其对子宫内膜异位症的临床诊断价值 被引量:1

摘要 目的检测子宫内膜异位症患者血清巨噬细胞迁移抑制因子(MIF)水平,分析MIF对子宫内膜异位症的诊断价值。方法本研究选取2021年12月至2022年12月期间于东莞市人民医院妇产科行手术治疗的子宫内膜异位症患者,将其作为实验组(n=68)。选取同期有慢性盆腔疼痛或不孕症的育龄期女性患者与同期在本院体检的健康女性各30例,并分别作为对照组与健康组。所有受试者于晨起时、实验组患者分别于术前(T0)、术后当天(T1)、术后第3天(T2)、术后第5天(T3)晨起时采集外周静脉血,检测各组MIF与癌抗原125(CA125)水平。并绘制受试者操作特征曲线(ROC曲线)分析诊断价值。结果血清MIF水平:实验组>对照组>健康组,差异有统计学意义(F=65.426,P<0.001);血清CA125水平:实验组>对照组>健康组,差异有统计学意义(F=85.743,P<0.001);实验组患者T2、T3的MIF与CA125水平均明显低于术前,差异有统计学意义(P<0.001);MIF诊断的AUC为0.899,敏感度与特异性分别为0.838、0.867;联合诊断的AUC为0.900,敏感度与特异性分别为0.779、0.967(P<0.05)。结论血清MIF检测可能有助于评估手术治疗对子宫内膜异位症病灶的清除情况;血清MIF水平对其诊断效能较高,且与联合CA125检测有助于提升诊断的特异性。 Objective To detect the level of serum macrophage migration inhibitory factor(MIF)in patients with endometriosis,and analyze the value of MIF in the diagnosis of endometriosis.Methods In this study,patients with endometriosis who underwent surgical treatment in the Obstetrics and Gynecology Department of Dongguan People's Hospital from December 2021 to December 2022 were selected as the experimental group(n=68).A total of 30 female patients of reproductive age with chronic pelvic pain or infertility and 30 healthy women in our hospital were selected as the control group and healthy group respectively.Peripheral venous blood was collected from all subjects in the morning and from the experimental group before surgery(T0),on the day after surgery(T1),on the third day after surgery(T2),and on the fifth day after surgery(T3),respectively,and the levels of MIF and cancer antigen 125(CA125)in each group were detected.The receiver operating characteristic curve(ROC curve)was drawn to analyze the diagnostic value.Results Serum MIF level:experimental group>Control group>Healthy group,the difference was statistically significant(F=65.426,P<0.001).Serum CA125 level:experimental group>Control group>Healthy group,the difference was statistically significant(F=85.743,P<0.001).The MIF and CA125 levels of T2 and T3 in the experimental group were significantly lower than those before surgery,and the difference was statistically significant(P<0.001).The AUC of MIF diagnosis was 0.899,and the sensitivity and specificity were 0.838 and 0.867,respectively.The AUC of combined diagnosis was 0.900,and the sensitivity and specificity were 0.779 and 0.967,respectively(P<0.05).Conclusion Serum MIF detection may be helpful to evaluate the clearance of endometriosis lesions after surgical treatment.Serum MIF level has high diagnostic efficacy,and combined with CA125 detection can help improve the specificity of diagnosis.
