颈动脉超声定量参数联合NLR、SII对2型糖尿病患者并发脑梗死的诊断价值

Diagnostic value of carotid ultrasound quantitative parameters combined with NLR and SII in type 2 diabetes patients with cerebral infarction

目的分析颈动脉超声定量参数联合中性粒细胞/淋巴细胞比值(NLR)、系统性免疫炎症指数(SII)对2型糖尿病患者并发脑梗死的诊断价值。方法选取2020年8月至2023年1月广德市人民医院收治的2型糖尿病患者并发脑梗死患者作为研究对象,将其命名为合并脑梗死组(n=86),另选同期本院收治的单纯性2型糖尿病患者为对照组(n=86)。比较两组的颈动脉超声定量参数[颈总动脉内膜中层厚度(IMT)、颈总动脉收缩期峰值血流速度(PSV)、舒张末期血流速度(EDV)]与NLR、SII水平;采用多因素Logistic回归分析2型糖尿病患者并发脑梗死的相关影响因素,并绘制ROC曲线分析颈动脉超声定量参数(IMT、PSV、EDV)联合NLR、SII对2型糖尿病患者并发脑梗死的预测价值。结果合并脑梗死组的IMT、EDV、NLR、SII水平均高于对照组(t=9.267、10.282、6.113、12.548、均P<0.05),PSV低于对照组,差异均具有统计学意义(t=8.144,均P<0.05);多因素Logistic回归分析显示,IMT升高(OR=1.696)、PSV降低(OR=1.846)、EDV升高(OR=1.943)、NLR水平升高(OR=2.323)及SII水平升高(OR=2.083)均是2型糖尿病患者并发脑梗死的独立危险因素(P<0.05);ROC曲线分析结果显示,IMT、PSV、EDV、NLR、SII水平联合检测优于单一检测(P<0.05)。结论颈动脉超声定量参数联合NLR、SII对2型糖尿病患者并发脑梗死具有较好的诊断价值。

Objective To analyze the diagnostic value of carotid ultrasound quantitative parame-ters combined with neutrophil/lymphocyte ratio(NLR)and systemic immunoinflammatory index(SII)in patients with type 2 diabetes complicated with cerebral infarction.Methods Patients with type 2 diabetes complicated with cerebral infarction admitted to Guangde People's Hospital from August 2020 to January 2023 were selected as the study object and named as the combined cerebral infarction group(n=86),and patients with simple type 2 diabetes admitted to our hospital during the same period were selected as the control group(n=86).Carotid artery ultrasound quantitative parameters[(common carotid intima media thickness(IMT),peak systol-ic blood velocity(PSV),end-diastolic blood velocity(EDV)]and NLR and SII were compared between the two groups.Multivariate Logistic regression was used to analyze the related influencing factors of cerebral infarction in patients with type 2 diabetes,and the ROC curve was drawn to analyze the predictive value of carotid artery ultrasound quantitative parameters(IMT,PSV,EDV)combined with NLR and SII in patients with type 2 diabetes complicated with cerebral infarction.Results The levels of IMT,EDV,NLR and SII in the combined cerebral infarction group were higher than those in the control group(t=9.267、10.282、6.113、12.548、P<0.05),and PSV was lower than those in the control group,the differences were statistically significant(t=8.144,P<0.05).Multivariate Logistic regression analysis showed that elevated IMT(OR=1.696),decreased PSV(OR=1.846),elevated EDV(OR=1.943),and elevated NLR(OR=2.323)and increased SII level(OR=2.083)were independent risk factors for cerebral infarction in type 2 diabetes patients(P<0.05).The ROC curve analysis results showed that the levels of IMT,PSV,EDV,NLR,SII respectively,and combined detection was superior to single detection(P<0.05).Conclusion Carotid ultrasound quantitative parameters combined with NLR and SII have good diagnostic value in type 2 diabetes patients complicated with cerebral infarction.
