CT在尿路结石化学成分分析中的应用及诊断价值

摘要 目的:分析电子计算机断层扫描(CT)在尿路结石化学成分分析中的应用及诊断价值。方法:选取2021年5—11月在九江市第一人民医院泌尿外科确诊为尿路结石患者89例,使用CT扫描机对患者进行扫描。使用结石红外光谱自动分析系统对结石标本进行分析。观察患者不同成分结石检出量,检测结石CT值,CT检查敏感度、特异度、准确度。结果:以红外光谱法检测为金标准,共检测出尿酸盐结石24颗,非尿酸盐结石99颗。CT测量结果显示,140、80 kV下尿酸盐结石CT值均低于非尿酸盐结石CT值,差异均有统计学意义(P<0.05);尿酸盐结石CT值差值、比值均低于非尿酸盐结石CT值差值、比值,差异均有统计学意义(P<0.05)。CT检查结果显示诊断敏感度为83.33%,特异度为98.99%,准确度为95.93%。结论:CT在尿路结石的化学成分分析中准确度较高,能够对成分不同的结石进行区分,使医师能够做出准确的判断,能够为临床诊断和指导治疗提供准确参考依据。 Objective:To analyze the application and diagnostic value of electronic computed tomography(CT) in the chemical composition analysis of urinary calculi.Method:A total of 89 patients diagnosed with urinary calculi in Urology Department of Jiujiang NO.1 People's Hospital from May to November 2021 were selected,and a CT scanner were used to scan the patients.Stone specimens were analyzed using an automatic infrared spectroscopy analysis system for stones.The detection amount of different components of stones in patients were observed,the CT value of stones,and the sensitivity,specificity,and accuracy of CT examination were detected.Result:Using infrared spectroscopy as the gold standard,a total of 24 urate stones and 99 non urate stones were detected.The CT measurement results showed that the CT values of urate stones at 140 and 80 kV were lower than those of non urate stones,and the differences were statistically significant(P<0.05);the difference and ratio of CT values for urate stones were lower than those for non urate stones,the differences were statistically significant(P<0.05).The CT examination results showed a diagnostic sensitivity of 83.33%,a specificity of 98.99%,and an accuracy of 95.93%.Conclusion:CT has high accuracy in the chemical composition analysis of urinary calculi,which can distinguish stones with different components,enabling physicians to make accurate judgments,and providing accurate reference basis for clinical diagnosis and treatment guidance.
