不同程度急性脊柱脊髓损伤患者血清神经营养因子及炎症因子水平表达的意义

The Significance of Serum Neurotrophic and Inflammatory Factor Levels in Patients with Acute Spinal Cord Injury of Different Severity

【目的】探讨不同程度急性脊柱脊髓损伤患者血清神经营养因子及炎症因子水平表达的意义。【方法】120例急性脊柱脊髓损伤的患者,依据神经功能分级分为完全损伤组(A组,52例)和不完全损伤组(B组,68例),另选取65例同期体检健康者作为对照组。比较三组对象血清B淋巴细胞瘤-2基因(Bcl-2)、Bcl-2相关X基因(Bax)、血清脑源性神经营养因子(BDNF)、神经营养素-3(NT-3)、神经营养素-4(NT-4)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平,分析各血清指标水平与患者损伤程度的相关性。【结果】A组和B组患者的血清Bcl-2水平显著高于对照组,Bax水平显著低于对照组(P<0.05);A组Bcl-2水平高于B组,Bax水平低于B组(P<0.05)。A组和B组患者血清BDNF、NT-3、NT-4、IL-6、TNF-α水平显著高于对照组(P<0.05),且A组各项指标水平高于B组(P<0.05)。Pearson相关分析显示,患者血清Bcl-2、BDNF、NT-3、NT-4、IL-6、TNF-α水平与脊柱脊髓损伤程度均呈正相关(P<0.05),Bax水平则呈与损伤程度负相关(P<0.05)。【结论】急性脊柱脊髓损伤患者血清炎症因子水平均升高,且随着损伤程度增加而升高,可作为判断损伤程度的指标之一。

【Objective】To explore the significance of serum neurotrophic and inflammatory factor levels in patients with acute spinal cord injuries of varying severity.【Methods】A total of 120 patients with acute spinal cord injury were divided into the complete injury group(Group A,52 cases)and the incomplete injury group(Group B,68 cases),based on the severity of neurological function injury.Additionally,65 healthy individuals undergoing physical examination in the same period were selected as the control group.Serum levels of B-cell lymphoma-2(Bcl-2),Bcl-2 associated X protein(Bax),brain-derived neurotrophic factor(BDNF),neurotrophin-3(NT-3),neurotrophin-4(NT-4),interleukin-6(IL-6),and tumor necrosis factor-alpha(TNF-α)were compared among the three groups.The correlation between these serum indicators and the degree of injury in patients was analyzed.【Results】The serum Bcl-2 level in both group A and B were significantly higher than that in the control group,while Bax level in both groups was significantly lower than that in the control group(P<0.05).Group A had higher Bcl-2 level and lower Bax level than group B(P<0.05).The levels of serum BDNF,NT-3,NT-4,IL-6,and TNF-αin patients of group A and group B were significantly higher than those in the control group(P<0.05);And group A had higher levels of serum BDNF,NT-3,NT-4,IL-6,and TNF-αthan group B(P<0.05).Pearson correlation analysis showed that serum levels of Bcl-2,BDNF,NT-3,NT-4,IL-6,and TNF-αin patients were positively correlated with the severity of spinal cord injury(P<0.05),while Bax level was negatively correlated(P<0.05).【Conclusion】The levels of serum inflammatory factors in patients with acute spinal cord injury are elevated with the severity of the injury,which makes them potential indicators for assessing the degree of injury.
