预康复护理在腹股沟疝日间手术患者中的应用

Application of pre-rehabilitation nursing in inguinal hernia patients undergoing day surgery

目的探讨术前预康复护理对腹股沟疝日间手术患者术后恢复质量的影响。方法选取2019年1月至2021年4月大连医科大学附属第一医院日间手术中心预约手术的339例腹股沟疝患者为研究对象,按照掷硬币法分为对照组(171例)和观察组(168例)。对照组予以常规术前护理,观察组采用预康复护理方案,除常规术前护理外,还包括运动预康复、营养支持、心理干预、疼痛及排便管理。比较两组患者的手术取消率、术后6 h的恢复情况、早期并发症发生情况和患者满意度。结果共有321例患者完成全程研究,其中对照组161例,观察组160例。观察组患者满意度、术后恢复质量评分中情感及日常生活活动能力评分及术后6 h数字评定量表(NRS)评分均高于对照组,差异有统计学意义(P<0.05)。两组患者其他术后早期并发症的发生率比较,差异无统计学意义(P>0.05)。结论对腹股沟疝日间手术患者术前实施预康复护理方案可有效提高术后恢复质量及患者满意度。

Objective To explore the effect of preoperative pre-rehabilitation nursing on the postoperative recovery quality of patients undergoing day surgery for inguinal hernia.Methods A total of 339 patients with inguinal hernia who had scheduled surgery at the Day Surgery Center of the First Affiliated Hospital of Dalian Medical University from January 2019 to April 2021 were selected as the research objects,and they were divided into control group(171 cases)and observation group(168 cases)according to coin toss method.The control group was given routine preoperative nursing,and the observation group was given pre-rehabilitation nursing,which included exercise pre-rehabilitation,nutritional support,psychological intervention,pain and defecation management in addition to routine preoperative nursing.The rate of operation cancellation,recovery 6 h after operation,early complications and patients'satisfaction were compared between the two groups.Results A total of 321 patients completed the whole study,including 161 patients in the control group and 160 patients in the observation group.The satisfaction,the scores of emotion and activity of daily living in postoperative recovery quality score and the score of numerical rating scale(NRS)at 6 h after surgery of patients in observation group were higher than those in control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of other early postoperative complications between the two groups(P>0.05).Conclusion Pre-rehabilitation nursing can effectively improve the quality of postoperative recovery and patients'satisfaction for patients undergoing day surgery for inguinal hernia.
