预立安宁整合照护计划对晚期癌症患者生命意义及家属生活质量的影响

Effect of advance hospice integrated care plan on life meaning and quality of life of family members of patients with advanced cancer

目的观察预立安宁整合照护计划对晚期癌症患者生命意义及家属生活质量的影响。方法选取2022年1月至2023年1月南昌大学第一附属医院肿瘤科收治的60例晚期癌症患者作为研究对象,按照随机数字表法分为对照组(30例)和试验组(30例),对照组按照常规方式护理,试验组在对照组的基础上实施预立安宁整合照护计划护理,比较两组患者的癌因性疲乏、患者生命意义、家属生活质量、护理满意度。结果干预后,试验组患者疲乏、活力、注意力、精力、日常生活活动及癌因性疲乏总评分均低于对照组,差异有统计学意义(P<0.05)。干预后试验组患者存在生命意义感、寻求生命意义感及生命意义总评分均高于对照组,差异有统计学意义(P<0.05)。干预后试验组患者家属躯体活动、精力水平、睡眠、社会隔离、情绪反应及诺丁汉健康调查问卷(NHP)总评分均低于对照组,差异有统计学意义(P<0.05)。干预后,试验组患者及家属的护理满意度高于对照组,差异有统计学意义(P<0.05)。结论预立安宁整合照护计划可以提高晚期癌症患者生命意义及家属生活质量。

Objective To observe the effects of advance hospice integrated care plan on the life meaning and quality of life of family members of patients with advanced cancer.Methods A total of 60 patients with advanced cancer admitted to department of oncology of the First Affiliated Hospital of Nanchang University,from January 2022 to January 2023 were selected as the study subjects and were divided into control group(30 cases)and treatment group(30 cases)according to the random number table method.The control group was given conventional method for nursing,and the treatment group was nursed with advance hospice integrated care plan on the basis of the control group.Cancer-related fatigue,life meaning,quality of life of family members and nursing satisfaction were compared between the two groups.Results After intervention,the scores of fatigue,vitality,attention,energy and activities of daily living and total score of cancer-related fatigue in treatment group were lower than those in control group,with statistically significant differences(P<0.05).After intervention,the scores of presence of sense of life meaning and seeking sense of life meaning and total score of life meaning in treatment group were higher than those in control group,with statistically significant differences(P<0.05).After intervention,the scores of physical activity,energy level,sleep,social isolation and emotional response and total score of Nottingham health questionnaire(NHP)of family members in treatment group were lower thanthose in control group,with statistically significant differences(P<0.05).After intervention,the nursing satisfaction of patients and their family members was higher in treatment group than that in control group,with a statistically significant difference(P<0.05).Conclusion Advance hospice integrated care plan can improve the life meaning and quality of life of family members of patients with advanced cancer.
