胰岛素注射相关皮下脂肪增生对糖尿病患者短期血糖波动的影响

摘要 目的探讨胰岛素注射相关皮下脂肪增生对糖尿病患者短期血糖波动的影响。方法选择2022年5月至2023年3月于我院进行胰岛素注射治疗且行动态血糖监测的30例胰岛素注射相关皮下脂肪增生1型糖尿病患者作为1型增生组,另选择同期同类治疗的30例无胰岛素注射相关皮下脂肪增生的1型糖尿病患者作为1型非增生组。选择同期我院胰岛素注射治疗且行动态血糖监测的30例胰岛素注射相关皮下脂肪增生2型糖尿病患者作为2型增生组,另选择同期同类治疗的30例无胰岛素注射相关皮下脂肪增生的2型糖尿病患者作为2型非增生组。统计患者血糖波动相关指标情况。结果1型增生组LAGE、MADE、MODD水平显著高于1型非增生组(P<0.05)。两组SD、CV、LI、CONGAn1h、CCONGAn4h比较,差异无统计学意义(P>0.05)。1型增生组短期血糖波动发生率水平(53.33%,16/30)显著高于1型非增生组(10.00%,3/30)(P<0.05)。2型增生组血糖波动相关指标与2型非增生组比较(P<0.05)。2型增生组短期血糖波动发生率水平(73.33%,22/30)显著高于2型非增生组(13.33%,4/30)(P<0.001)。结论胰岛素注射相关皮下脂肪增生对1型和2型糖尿病患者均对血糖指标有影响,可增加短期血糖波动风险,对于血糖控制较为不利,需积极防控该类脂肪增生发生发展。 Objective To investigate the effect of insulin injection-related subcutaneous fat hyperplasia on short-term blood glucose fluctuation in diabetic patients.Methods 30 cases of insulin injection-related subcutaneous fat hyperplasia type 1 diabetes mellitus patients who were treated with insulin injection and underwent ambulatory glucose monitoring from May 2022 to March 2023 in our hospital were selected as the type 1 hyperplasia group,and 30 cases of type 1 diabetes mellitus patients without insulin injection-related subcutaneous fat hyperplasia treated in the same category during the same period were also selected as the type 1 non-hyperplasia group.Select 30 cases of insulin injection-related subcutaneous fat hyperplasia type 2 diabetic patients treated with insulin injection and dynamic glucose monitoring in the same period of time as the type 2 hyperplasia group,and select 30 cases of type 2 diabetic patients without insulin injection-related subcutaneous fat hyperplasia treated in the same period of time as the type 2 non-hyperplasia group.The patients'indicators related to blood glucose fluctuation were counted.Results The levels of LAGE,MADE,and MODD in the type 1 hyperplasia group were significantly higher than those in the type 1 nonhyperplasia group(P<0.05).Comparison of SD,CV,LI,CONGAn1h,CCONGAn4h between the two groups,the difference was not statistically significant(P>0.05).The level of incidence of short-term glucose fluctuation in the type 1 hyperplasia group(53.33%,16/30)was significantly higher than that of the type 1 nonhyperplasia group(10.00%,3/30)(P<0.05).The blood glucose in the type 2 hyperplasia group fluctuation related indexes were compared with type 2 nonhyperplasia group(P<0.05).The level of incidence of short-term glucose fluctuation in type 2 hyperplasia group(73.33%,22/30)was significantly higher than that in type 2 nonhyperplasia group(13.33%,4/30)(P<0.001).Conclusions Insulin injection-related subcutaneous fat hyperplasia affects glycemic indexes in both type 1 and type 2 diabetes mellitus patients,increases the risk of short-term glycemic fluctuations,which is harmful to glycemic control,so it is necessary to actively prevent and control the development of this type of fat hyperplasia.
