院内-居家过渡的老年冠心病患者多重用药现状及用药依从性影响因素分析

Analysis of the current status of polypharmacy and medication adherence and influencing factors in geriatric patients with coronary heart disease during hospital-home transition

目的探讨院内-居家过渡阶段的老年冠心病患者多重用药现状及其用药依从性影响因素。方法采用便利抽样方法,利用自制问卷及Morisky用药依从性量表对200例就诊于北京某三甲医院心内科次日出院的老年冠心病患者进行调查,应用χ2检验、Logistic回归分析影响用药依从性的因素。结果老年冠心病患者用药依从不良率为40.5%。Logistic回归分析显示家庭人均月收入、对医嘱的重要性认知、服药执行情况、药物素养、是否出现过副作用、居住情况是用药依从性的影响因素(P<0.05)。结论院内-居家过渡的老年冠心病患者用药依从性处于较低水平,仍有待进一步提高。临床医务工作者应在院内加强老年人用药相关的健康宣教,协助老年人正确理解用药知识,进一步提升其居家用药依从性。

Objective To explore the current situation of polypharmacy in elderly patients with coronary heart disease and the factors affecting medication adherence during hospital-home transition.Methods A self-made questionnaire was used to investigate 200 elderly patients with coronary heart disease in the Department of Cardiology of a Class III Grade hospital in Beijing,useχ2 and Logistic regression analysis the factors affecting medication adherence.Results The patients with poor medication adherence accounted for 40.5%.Logistic regression analysis showed that family per capita monthly income,understanding the importance of doctor's advice,medication implementation,drug literacy,had any side effect occurred,and residential situation were the influencing factors of medication adherence(P<0.05).Conclusion Medication adherence of elderly patients with coronary heart disease is at a low level and still needs to be further improved.Clinical medical workers should strengthen the health education related to medication for the elderly in hospital,help patients accurately understand medication knowledge,ensure the efficacy and safety of drug therapy,and further improve their medication adherence at home.
