探讨床旁超声在ICU急性肾损伤(AKI)患者CRRT容量管理中的临床价值

摘要 目的探究与分析床旁超声在AKI患者CRRT容量管理中的临床价值。方法按照随机数字表法对ICU内收治的AKI患者80例进行分组,每组各40例,两组患者均采用同样的CRRT方法治疗,对照组按照患者中心静脉压的变化指标对液体平衡进行调节,观察组加用床旁超声技术,对比两组临床效果。结果观察组治疗后与对照组治疗后相比,CRP、SCr、BUN均较低,尿量、TAPSE及IVCRVI值均较高,IVC值较低(P<0.05),观察组与对照组相比,尿量恢复时间及ICU治疗时间均较短,不良心血管事件发生率较低(P<0.05),差异有统计学意义。结论床旁超声应用在AKI患者CRRT容量管理中可获得更好的临床治疗效果,有效改善肾功能指标以及炎症反应,降低不良心血管事件发生风险,预后表现更好。 Objective To explore and analyze the clinical value of bedside ultrasound in the volume management of CRRT in AKI patients.Methods According to the random number table method,80 patients with AKI admitted to the ICU were divided into two groups,40 cases in each group.Both groups were treated with the same CRRT method,the control group was adjusted according to the change index of central venous pressure,and the observation group was added with bedside ultrasound technology.The clinical effects of the two groups were compared.Results Compared with the control group after treatment,the CRP,SCr and BUN in the observation group were lower,the urine volume,TAPSE and IVCRVI values were higher,and the IVC value was lower(P<0.05).Compared with the control group,the urine volume recovery time and ICU treatment time in the observation group were shorter,and the incidence of adverse cardiovascular events was lower(P<0.05).The difference was statistically significant.Conclusion The application of bedside ultrasound in the volume management of CRRT in ICU patients with acute kidney injury(AKI)can achieve better clinical treatment effect,effectively promote the improvement of renal function indicators and inflammatory response,reduce the risk of adverse cardiovascular events,and better prognosis.
