婚姻状况对胰腺癌患者生存情况的影响研究

Influence of marital status on survival of pancreatic cancer patients

目的探讨不同婚姻状况对胰腺癌患者肿瘤特异性生存期(CSS)的影响。方法从SEER数据库获取2004-2016年胰腺癌患者临床资料,按照不同婚姻状态、年龄、TNM分期、手术情况将患者进行分组,采用多变量COX回归模型分析婚姻状况对胰腺癌患者CSS的影响。结果不同婚姻状况、年龄、种族、TNM分期、Grade分级、手术情况患者中位CSS比较,差异有统计学意义(P<0.01)。婚姻状况、年龄、Grade分级、TNM分期、手术情况为胰腺癌患者CSS的独立影响因素(P<0.01)。在<60、≥60岁年龄段中,婚姻状况是胰腺癌患者CSS的独立影响因素(P<0.05)。在未手术组中,婚姻状况是胰腺癌患者CSS的独立影响因素(P<0.05)。在手术组中,婚姻状况不是胰腺癌患者CSS的独立影响因素(P>0.05)。在Ⅲ、Ⅳ期组中,婚姻状况是胰腺癌患者CSS的独立影响因素(P<0.05)。在Ⅰ、Ⅱ期组中,婚姻状况不是胰腺癌患者CSS的独立影响因素(P>0.05)。结论婚姻状况与胰腺癌患者预后相关,已婚患者组CSS优于未婚、不良婚姻患者。

Objective To explore the influence of different marital status on cancer-specific survival(CSS)of pancreatic cancer patients.Methods The clinical data of pancreatic cancer patients from 2004 to 2016 were obtained from the SEER database.The patients were grouped according to different marital status,age,TNM stage and surgical conditions.Multivariate COX regression model was used to analyze the influence of marital status on CSS of pancreatic cancer patients.Results There were statistically significant differences in median CSS among patients with different marital status,age,race,TNM stage,Grade classification,and surgical status(P<0.01).Marital status,age,Grade classification,TNM stage and operation were independent influencing factors of CSS in pancreatic cancer patients(P<0.01).In the age group of<60 and≥60 years old,marital status was an independent influencing factor of CSS in pancreatic cancer patients(P<0.05).In the non-surgical group,marital status were independent influencing factors for CSS in pancreatic cancer patients(P<0.05).In the surgical group,marital status was not independent factor affecting CSS in pancreatic cancer patients(P>0.05).In the phaseⅢandⅣgroups,marital status was an independent influencing factor of CSS in pancreatic cancer patients(P<0.05).In the stageⅠandⅡgroups,marital status was not an independent influencing factor for CSS in pancreatic cancer patients(P>0.05).Conclusion Marital status is related to the prognosis of pancreatic cancer patients.The CSS of married patients is better than that of unmarried and poor married patients.
