彩色超声结合乳腺钼靶在早期乳腺癌筛查中的临床价值

摘要 目的探讨彩色超声结合乳腺钼靶检查对早期乳腺癌筛查的意义。方法方便选择2020年1月—2023年6月盐城市大丰人民医院收治的疑似早期乳腺癌患者82例为研究对象,所有患者均经手术病理确诊,术前分别给予彩色超声、乳腺钼靶、两项结合检查,对不同检查方法的结果进行比较。结果两项结合检查灵敏度、特异度高于单项检查,差异有统计学意义(P<0.05)。以病理诊断结果作为“金标准”,对于不同直径病灶的检出情况如下:直径<1.0 cm14例、1.0 cm<直径<1.6 cm有40例、直径1.6~2.0 cm有6例,联合检测对直径≤1.0 cm、1.0 cm<直径<1.6 cm病灶的诊断符合率分别为92.86%、97.50%,高于单一检测,差异有统计学意义(χ^(2)=11.083、10.798,P<0.05),单一及联合检测对直径1.6~2.0 cm病灶的诊断符合率比较,差异无统计学意义(P>0.05)。结论彩色超声结合乳腺钼靶检查用于早期乳腺癌筛查的结果可靠性更高。 Objective To explore the significance of color ultrasound combined with mammography in early breast cancer screening.Methods A total of 82 patients with suspected early breast cancer admitted to Dafeng People's Hospital of Yancheng City from January 2020 to June 2023 were selected as the study objects conveniently.All patients were confirmed by surgery and pathology.Before surgery,color ultrasound,mammography and combined mammography were given respectively,and the results of different detection methods were compared.Results The sensitivity and specificity of the two combined tests were higher than those of the single test,and the differences were statistically significant(P<0.05).Using pathological diagnosis results as the"gold standard",the detection of lesions with different diameters is as follows:14 cases with a diameter≤1.0 cm,40 cases with 1.0 cm<diameter<1.6 cm,and 6 cases with a diameter of 1.6~2.0 cm.The diagnostic accuracy of combined testing for lesions with a diameter≤1.0 cm and 1.0 cm<diameter<1.6 cm were 92.86%and 97.50%,respectively,higher than those of single testing,and the differences were statistically significant(χ^(2)=11.083,10.798,P<0.05),but there was no statistically significant difference in the diagnostic accuracy of single and combined detection for lesions with a diameter of 1.6~2.0 cm(P>0.05).Conclusion The results of color ultrasound combined with mammography for early breast cancer screening are more reliable.
