中医叙事医学实践中的临床共情

Clinical Empathy in the Practice of Traditional Chinese Medicine Narrative Medicine

针对当前国内医疗亟待本土化叙事医学实践的助力,及提高医患间临床共情的实际需要,基于临床共情的四个维度:情感、道德、认知、行为,探讨并阐述中医诊疗中的叙事医学实践及临床共情,分析中医临床共情四个维度间的内在联系,并总结其整全性、灵动性的总体特点。认为中医临床诊疗中的医学人文实践不仅体现出叙事医学的特点和优势,也以更符合中国人性格特点与文化习俗的方式进行临床共情,有利于医患间更好地建立信任,弥合分歧,可为叙事医学实践建立医患共情提供借鉴。

In response to the urgent need for localized narrative medicine practice in current domestic healthcare and the practical need to improve clinical empathy between doctors and patients,this article explores and elaborates on narrative medicine practice and clinical empathy in the process of Traditional Chinese Medicine(TCM)diagnosis and treatment based on four dimensions of clinical empathy:emotional dimension,moral dimension,cognitive dimension,and behavioral dimension,analyzes the internal connections among the four dimensions of clinical empathy in TCM,and summarizes its overall characteristics of integrity and flexibility.The authors believe that the medical humanistic practice in TCM clinical diagnosis and treatment not only reflect the characteristics and advantages of narrative medicine,but also engage in clinical empathy in a manner that aligns with the personality characteristics and cultural customs of Chinese people,which is conducive to building trust between doctors and patients,bridging differences,and providing reference for the establishment of doctor-patient empathy in narrative medicine practice.
