彩色多普勒血流成像和超声弹性成像检查对乳腺良恶性肿块的鉴别诊断价值

Value of color Doppler flow imaging and ultrasonic elastography in the differential diagnosis of benign and malignant breast masses

目的 比较彩色多普勒血流成像(CDFI)和超声弹性成像(UE)检查对乳腺良恶性肿块的鉴别诊断价值。方法 选取98例乳腺肿块患者,均接受CDFI和UE检查,以病理检查结果为金标准,分析CDFI和UE检查对乳腺恶性肿块的诊断价值。结果98例患者共112个乳腺肿块,均予以穿刺或手术活检,其中良性肿块64个,恶性肿块48个。UE检查诊断乳腺恶性肿块的灵敏度、特异度、准确度、阳性预测值、阴性预测值分别为91.67%、90.62%、91.07%、88.00%、93.55%,分别高于CDFI检查的77.08%、76.56%、76.79%、71.15%、81.67%。结论 CDFI和UE检查均可作为乳腺恶性肿块的诊断方法,但UE检查的诊断价值更高,能够更加准确地判断乳腺肿块的良恶性,值得临床推广。

Objective To compare the value of color Doppler flow imaging(CDFI) and ultrasonic elastography(UE)in the differential diagnosis of benign and malignant breast masses.Method A total of 98 patients with breast masses were selected and all patients received CDFI and UE examination.The diagnostic value of CDFI and UE in malignant breast masses was analyzed by pathological results as the golden standard.Result A total of 112 breast masses were found in 98 patients,all of which underwent biopsy or surgical biopsy.The results confirmed that 64 were benign masses and 48 were malignant masses.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of UE in the diagnosis of malignant breast masses were 91.67%,90.62%,91.07%,88.00% and 93.55%,which were higher than 77.08%,76.56%,76.79%,71.15% and 81.67% of CDFI.Conclusion Both CDFI and UE can be used as diagnostic methods for malignant breast masses,but UE has higher diagnostic value and can more accurately judge the benign and malignant breast masses,which is worthy of clinical promotion.
