自我管理个体护理模式对血液透析患者动静脉内瘘术后护理质量及心理状态的影响

The Effect of Self-Managed Individual Care Model on the Quality of Care and Psychological Status of Hemodialysis Patients After Arteriovenous Endovascular Fistula Surgery

目的探讨自我管理个体护理模式对血液透析患者动静脉内瘘术后护理质量及心理状态的影响。方法选取福建医科大学附属第一医院在2021年3月至2023年3月收治的120例血液透析行动静脉内瘘术的患者,将其以双盲法随机分为研究组和对照组,均为60例患者,对照组以常规管理模式干预,研究组实施自我管理个体护理模式,对两组护理质量及心理状态、并发症发生率等进行比较。结果研究组术后护理质量评分高于对照组(P<0.05);干预后,研究组HAMA(10.34±1.37)分和HAMD评分(9.46±1.33)分均低于对照组(P<0.05);研究组内瘘功能障碍发生率5.00%低于对照组18.33%(χ^(2)=5.175,P=0.023);研究组动静脉内瘘术后并发症发生率3.33%低于对照组20.00%(P<0.05)。结论对血液透析患者动静脉内瘘术后实施自我管理个体护理模式效果肯定,可有效提高术后护理质量,改善负面情绪,减少相关并发症的发生。

Objective To explore the impact of self-management individual nursing model on the quality of care and psychological status of hemodialysis patients after arteriovenous fistula surgery.Methods A total of 120 patients with hemodialysis induced venous fistula admitted to our hospital from March 2021 to March 2023 were randomly divided into a study group and a control group using a double-blind method,with 60 patients in each group.The control group received routine management intervention,while the study group implemented a self-managed individual care model.The quality of care,psychological status,and incidence of complications were compared between the two groups.Results The postoperative quality of care score of the study group was higher than that of the control group(P<0.05);after the intervention,the HAMA(10.34±1.37)score and HAMD score(9.46±1.33)of the study group were lower than that of the control group(P<0.05);the incidence of endovascular fistula dysfunction of the study group was lower than that of the control group of 5.00%and 18.33%,respectively(χ^(2)=5.175,P=0.023);The incidence of postoperative complications of arteriovenous endovascular fistula in the study group was 3.33%lower than 20.00%in the control group(P<0.05).Conclusions The implementation of self-management individual nursing model for hemodialysis patients with arteriovenous fistula after surgery has a positive effect,can effectively improve the quality of postoperative care,improve negative emotions,and reduce the occurrence of related complications.
