杞菊地黄汤联合通窍明目针法治疗糖尿病视网膜病变的临床观察

Qiju Dihuang Decoction Combined with Tongqiao Mingmu Acupuncture in the Treatment of Diabetic Retinopathy

目的 探讨杞菊地黄汤联合通窍明目针法治疗糖尿病视网膜病变(DR)的临床效果。方法 将98例非增殖期DR患者随机分为对照组49例和试验组49例。对照组予以常规治疗及羟苯磺酸钙胶囊,试验组在对照组基础上予以杞菊地黄汤联合通窍明目针法治疗。2组患者治疗时间均为8周。比较2组临床疗效,视网膜渗漏总面积及毛细血管无灌注区总面积,黄斑区视网膜平均厚度(CAT)、体积(CV)及中央区厚度(CST)。结果 治疗后,试验组总有效率为93.88%(46/49),高于对照组的75.51%(37/49)(P<0.05)。与对照组比较,试验组治疗后视网膜渗漏总面积及毛细血管无灌注区总面积较低,黄斑区CAT、CV及CST较低(P<0.05)。结论 杞菊地黄汤联合通窍明目针法能明显改善DR患者眼底视网膜微血管病变,缓解黄斑区视网膜水肿,临床疗效显著。

Objective To investigate the clinical efficacy of Qiju Dihuang decoction combined with Tongqiao Mingmu acupuncture in the treatment of diabetic retinopathy(DR).Methods Ninety-eight patients with non proliferative DR were randomly divided into control group of 49 cases and treatment group of 49 cases.The control group was treated with routine treatment and calcium dobesilate capsules,and on the basis of the control group,the treatment group was treated with Qiju Dihuang decoction combined with Tongqiao Mingmu acupuncture based on midnight-noon ebb-flow acupoint selection.The treatment time of the two groups were 8 weeks.The clinical efficacy,total area of retinal leakage and total area of capillaries without perfusion,cube average thickness(CAT),cube volume(CV) and central sufield thickness(CST) in macular region were compared between the 2 groups.Results After treatment,the total effective rate of experimental group was 93.88%(46/49),which was higher than that of control group(75.51%,37/49)(P<0.05).Compared with the control group,the total area of retinal leakage and the total area of capillary non-perfusion area were lower in experimental group after treatment,and the CAT,CV and CST in macular area were lower(P<0.05).Conclusion Qiju Dihuang decoction combined with Tongqiao Mingmu acupuncture can significantly improve the retinal microangiopathy of patients with DR,alleviate the retinal edema in the macular area,and has significant clinical effect.
