热奄包联合穴位贴敷在股骨骨折术后护理观察

External Application of Fomentation Package Combined with Acupoint Application in Postoperative Nursing of Femoral Fracture

目的 探讨热奄包联合穴位贴敷在股骨骨折术后护理中的应用价值。方法 选取股骨骨折患者80例,以护理方法差异分为对照组40例和观察组40例。对照组术后以常规护理干预,观察组术后加以热奄包外敷联合穴位贴敷护理干预。比较2组术后肠鸣音恢复时间、首次排气时间、首次排便时间、术后3 d排便率、术后(1 d、3 d)患肢外径、术后(1 d、2 d、3 d)视觉模拟评分(VAS)及术后并发症。结果 观察组术后肠鸣音恢复时间、首次排气时间、首次排便时间均短于对照组,术后3 d排便率高于对照组(P<0.05);观察组术后1 d、2 d的VAS评分低于对照组(P<0.05),但2组术后3 d的VAS评分差异无统计学意义(P>0.05);2组术后1 d患肢外径均较术前增加,观察组术后1 d、3 d患肢外径均小于对照组(P<0.05);观察组术后总并发症发生率低于对照组(P<0.05)。结论 股骨骨折术后辅以热奄包联合穴位贴敷干预,可有效缓解患肢肿胀,缓解疼痛,促进胃肠道功能恢复,降低并发症发生率,值得临床推广。

Objective To discusses the application value of fomentation package and acupoint application in postoperative nursing of femoral fracture.Methods Eighty patients with femoral fracture who underwent surgical treatment were divided into control group and observation group according to the difference of nursing methods,40 patients in each group.The control group was treated with routine postoperative nursing,and the observation group was treated with Hot external application of fomentation package and acupoint application.The recovery of gastrointestinal function(recovery time of bowel sounds,first exhaust time,first defecation time,and defecation rate 3 days after operation) were compared between the two groups.The external diameter of the affected limb were compared on the 1st and 3rd day after operation.The visual analogue scale(VAS) and postoperative complications were evaluated on the first day,the second day and the third day after operation.Results The postoperative bowel sound recovery time,the first exhaust time and the first defecation time in the observation group were shorter than those in the control group,and the defecation rate at 3 days after operation was higher than that in the control group(P<0.05).The VAS score of the observation group was lower than that of the control group(1and 2 days after surgery)(P<0.05),but there was no significant difference between the two groups(P>0.05).The outside diameter of the affected limb in 2 groups was increased 1 day after surgery,and the outside diameter of the affected limb in the observation group was smaller than that in the control group 1 day and 3 days after surgery(P<0.05).The incidence of postoperative complications in observation group was lower than that in control group(P<0.05).Conclusion The total intervention of external application of fomentation package and acupoint application after the operation of femoral fracture can effectively relieve the swelling of the affected limb,relieve pain,promote the recovery of gastrointestinal function,and reduce the incidence of complications,which is worthy of clinical promotion.
