老年慢性阻塞性肺疾病稳定期患者遵医行为的影响因素

Influencing Factors of Compliance Behavior in Elderly Patients with Stable Chronic Obstructive Pulmonary Disease

目的了解老年慢性阻塞性肺疾病(COPD)稳定期患者遵医行为影响因素,并采取相对应的护理干预策略。方法回顾性选取2019年3月至2023年5月于郑州大学第五附属医院接受诊疗的老年COPD稳定期患者84例作为研究对象,采用一般资料调查表和遵医行为调查问卷对患者进行调查,并采用单因素分析和多因素logistic回归分析对老年COPD稳定期患者遵医行为影响因素进行分析。结果遵医行为调查问卷结果显示,遵医患者39例,占比46.43%,未遵医患者45例,占比53.57%。其中婚姻状况、受教育程度、家庭年收入、现居地、工作情况、病程、每年住院次数、健康教育和咨询、心理弹性的患者比较遵医与未遵医占比差异具有统计学意义(P<0.05)。多因素logistic回归分析结果显示,患者婚姻状况、受教育程度、家庭年收入、每年住院次数、健康教育和咨询情况、心理弹性为老年COPD稳定期患者遵医行为的影响因素(P<0.05)。结论患者婚姻状况、受教育程度、家庭年收入、每年住院次数、健康教育和咨询情况、心理弹性为老年COPD稳定期患者遵医行为的影响因素,临床上可根据上述因素制定相应的干预策略。

Objective To understand the influencing factors of compliance behavior of elderly patients with chronic obstructive pulmonary disease(COPD)at stable stage and take corresponding nursing intervention strategies.Methods A total of 84 elderly patients with stable COPD who were treated in the Fifth Affiliated Hospital of Zhengzhou University from March 2019 to May 2023 were retrospectively selected as the research objects.The general information questionnaire and the compliance behavior questionnaire were used to investigate the patients,and univariate and multivariate logistic regression analysis were used to analyze the influencing factors of the compliance behavior of elderly patients with stable COPD.Results The questionnaire survey results of compliance behavior showed that there were 39 patients with compliance,accounting for 46.43%,and 45 patients without compliance,accounting for 53.57%.Among them,there were statistically significant differences in the proportion of patients with different marital status,education,annual family income,current residence,work situation,disease duration,number of hospitalizations per year,health education and counseling,and psychological resilience between compliance and non-compliance(P<0.05).Multivariate logistic regression analysis showed that marital status,education level,annual family income,number of hospitalizations per year,health education and counseling,and psychological resilience were the influencing factors of compliance behavior in elderly patients with stable COPD(P<0.05).Conclusion Patients’marital status,education level,annual family income,the number of hospitalizations per year,health education and counseling,and psychological resilience are the influencing factors of compliance behavior of elderly patients with stable COPD.In clinic,corresponding intervention strategies can be formulated according to the above factors.
